摘要
目的探讨术中冷冻病理切片在甲状腺微小乳头状癌中的诊断价值。方法选取2021年10—12月抚州市第一人民医院收治的60例甲状腺微小乳头状癌患者为研究对象。收集其病理样本,均进行术中冷冻切片诊断,以术后石蜡包埋切片诊断为金标准,分析术中冷冻病理切片诊断甲状腺微小乳头状癌中的价值。结果两种切片检查方式乳头结构、细胞核异常、细胞间质异常、钙化及其他异常结构检出率比较差异无统计学意义。两种切片检查方式诊断准确率、误诊率、漏诊率比较差异无统计学意义。术中冷冻切片诊断时间为(25.36±5.31)min,短于术后石蜡包埋切片的(1986.97±410.31)min,差异有统计学意义(P<0.05)。两种切片检查方式喉前淋巴结、气管旁淋巴结、气管前淋巴结、中央区淋巴结检出率比较差异无统计学意义。结论在甲状腺微小乳头状癌诊断中,术中冷冻切片病理诊断与术后石蜡包埋切片诊断一致性理想,但仍存在误诊、漏诊情况。
Objective To explore the diagnostic value of intraoperative frozen section pathology in papillary thyroid carcinoma.Methods 60 patients with papillary thyroid carcinoma admitted to Fuzhou First People's Hospital from October to December 2021 were selected as the study subjects.Pathological samples were collected and all underwent intraoperative frozen section diagnosis,the postoperative paraffin embedded section diagnosis was used as the gold standard,and the value of intraoperative frozen section pathology diagnosis in papillary thyroid carcinoma was analyzed.Results There were no statistically significant differences in the detection rates of nipple structure,nuclear abnormalities,interstitial abnormalities,calcification,and other abnormal structures between the two slice examination methods.There were no significant differences in the diagnostic accuracy,misdiagnosis rate,and missed diagnosis rate between the two slice examination methods.The diagnostic time of intraoperative frozen section was(25.36±5.31)min,which was shorter than(1986.97±410.31)min of the postoperative paraffin embedded section,and the difference was statistically significant(P<0.05).There were no significant differences in the detection rate of prelaryngeal lymph nodes,paratracheal lymph nodes,pretracheal lymph nodes and central lymph nodes between the two slice examination methods.Conclusion In the diagnosis of papillary thyroid carcinoma,the consistency between intraoperative frozen section pathological diagnosis and postoperative paraffin embedded section diagnosis is ideal,but there are still cases of misdiagnosis and missed diagnosis.
