摘要
目的观察电针对大鼠神经病理性疼痛及谷氨酸受体1(GluR1)表达的影响,探讨电针镇痛效应机制。方法将36只SD大鼠随机分为假模组、模型组、电针组,每组12只。前两组采用脊神经结扎(SNL)的方法建立大鼠神经痛模型,假模组仅分离脊神经不结扎。电针组在造模后7 d电针干预大鼠患侧“足三里”“环跳”穴,其他两组仅给予固定,不予治疗,每次30 min,1次/d,连续7 d。在造模前1 d及造模后第3、5、7、10、12、14天分别测定大鼠机械缩足反射阈值(MWT)及热缩足反射潜伏期(TWL),术后15 d处死大鼠,取大鼠L4~L6段腰膨大脊髓,用免疫组化及Western blot检测GluR1蛋白的表达,采用逆转录-聚合酶链反应(RT-PCR)检测GluR1-mRNA的表达。结果与假模组相比,模型组大鼠痛阈值明显降低,差异有统计学意义(P<0.01),出现痛觉过敏;电针干预后,电针组较模型组痛阈值明显升高,差异有统计学意义(P<0.01),与假模组相比,模型组与电针组GluR1蛋白及mRNA表达水平明显增高,差异有统计学意义(P<0.01),与模型组相比,电针组GluR1阳性蛋白表达水平明显下调,差异有统计学意义(P<0.01)。结论电针“足三里”“环跳”穴可以缓解大鼠神经病理性疼痛,该作用可能与其下调大鼠脊髓背角AMPA受体GluR1的表达密切相关。
Objective To observe the effect of electro-acupuncture on neuropathic pain and glutamate receptor 1(GluR1)expression in rats,and to explore the mechanism of electro-acupuncture analgesia.Methods A total of 36 SD rats were randomly divided into sham model group,model group and electro-acupuncture group,with 12 rats in each group.The first two groups used the method of spinal nerve ligation(SNL)to establish the rat model of neuralgia,while the sham model group only separated the spinal nerve without ligation.The electro-acupuncture group intervened at Zusanli and Huantiao acupoints on the affected side of rats 7 days after modeling.The remaining groups were only fixed without treatment,30 min each time,once a day for 7 days.The mechanical foot reflex threshold(MWT)and thermal foot reflex latency(TWL)were measured 1 day before and 3,5,7,10,12 and 14 days after the establishment of the model.Rats were killed 15 days after the operation.L4-L6 lumbar enlarged spinal cord was taken from the rats.The expression of GluR1-mRNA was detected by immunohistochemistry and Western blot.The expression of GluR1 gene was detected by reverse transcription-pdymerase chain reaction(RT-PCR).Results Compared with the sham model group,the pain threshold of the model group was significantly lower(P<0.01),and there was hyperalgesia.After electro-acupuncture intervention,the pain threshold of the electro-acupuncture group was significantly higher than that of the model group(P<0.01).Compared with the sham model group,the GluR1 protein and mRNA expression water of the model group and the electro-acupuncture group were significantly higher(P<0.01).Compared with the model group,the expression level of GluR1 positive protein in the electro-acupuncture group decreased significantly(P<0.01).Conclusion Electro-acupuncture at Zusanli and Huanjuan can relieve neuropathic pain in rats,which may be closely related to down-regulation of AMPA receptor GluR1 expression in the dorsal horn of spinal cord.
