摘要
目的:探讨养元解郁汤联合帕罗西汀对产后抑郁患者心理状态及雌激素水平的影响。方法:将120例产后抑郁患者按照随机数字表法分为观察组和对照组,每组各60例。对照组给予帕罗西汀治疗,观察组在对照组的基础上加用养元解郁汤。比较两组患者的临床疗效及治疗前后抑郁自评量表(self-rating depression scale,SDS)评分、中医证候积分,检测两组患者治疗前后激素水平[雌二醇(estradiol,E_2)、孕酮(progesterone,P)]以及脑内神经递质[神经递质(neurotranmitters,INH)、5-羟色胺(5-hydroxy tryptamin,5-HT)、多巴胺(dopamine,DA)、去甲肾上腺素(norepinephrine,NE)]的变化情况。结果:对照组有效率为85.00%,观察组有效率为96.67%,两组有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后SDS评分及中医证候积分均低于本组治疗前,且观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后E_2水平高于本组治疗前,P水平低于本组治疗前,观察组治疗后E_2水平高于对照组,P水平低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后5-HT、NE含量均高于本组治疗前,观察组治疗后高于对照组,差异具有统计学意义(P<0.05)。结论:养元解郁汤联合帕罗西汀治疗产后抑郁,可明显改善患者心理状态、雌激素水平和脑内神经递质含量。
Objective:To explore Decoction for Nourishing Primordial Qi and Relieving Depression combined with Paroxetine on psychological state and estrogen level of postpartum depression patients.Methods:A total of 120 patients with postpartum depression were randomly divided into the observation group and the control group,with 60 cases in each group.The control group was treated with Paroxetine,while the observation group was treated with Decoction for Nourishing Primordial Qi and Relieving Depression on the basis of the control group.The clinical efficacy,SDS score and TCM syndrome score before and after treatment were compared between the two groups.The changes of levels of estradiol(E2),progesterone(P)and brain neurotransmitters(INH),5-hydroxy tryptamin(5-HT),dopamine(DA)and norepinephrine(NE)before and after the treatment were measured.Results:The effective rate of the control group was 85.00%,and that of the observation group was 96.67%.The difference between the two groups was statistically significant(P<0.05).The scores of SDS and TCM syndromes in the two groups after the treatment were lower than those before the treatment,and those in the observation group were lower than those in the control group,all the differences were statistically significant(P0.05).After the treatment,E2 level in each group was higher than that of the same group before the treatment,P level was lower than that of the same group before the treatment.E2 level in the observation group was higher than that in control group,P level was lower than that in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Decoction for Nourishing Primordial Qi and Relieving Depression has a remarkable efficacy on postpartum depression,can significantly improve the mental state,estrogen level and neurotransmitter level in the brain of patients.
