运用Logistic回归联合ROC曲线评估miR-19b-3p对抑郁症疗效的预测价值

摘要 目的运用Logistic回归联合ROC曲线评估微小核糖核酸-19b-3p(microRNA-19b-3p,miR-19b-3p)对抑郁症疗效的预测价值。方法选取本院收治的抑郁症患者112例为研究对象(病例组),另选取同期来本院进行体检的健康志愿者60例(作为对照组)。采用实时荧光定量聚合酶链式反应(polymerase chain reaction,PCR)法检测所有受试者外周血清miR-19b-3p表达水平,并比较治疗前2组血清miR-19b-3p表达水平及病例组治疗前后血清miR-19b-3p表达水平变化。根据治疗效果将病例组分为预后良好组和预后不良组,比较预后良好组与预后不良组血清miR-19b-3p表达水平及其他因素。采用Logistic回归分析法明确影响抑郁症患者预后不良的危险因素,并绘制受试者工作特征(receiver operating characteristical,ROC)曲线分析治疗前后血清miR-19b-3p表达水平对抑郁症患者预后不良的预测价值。结果治疗前,病例组血清miR-19b-3p水平高于对照组(P<0.05),治疗后,病例组血清miR-19b-3p水平为低于治疗前(P<0.05)。预后良好组抑郁症家族史(阳性)、治疗依从性差、家庭社会支持情况差、与邻里关系差、重大生活事件占比,发作次数、治疗前后血清miR-19b-3p水平高于预后良好组(P<0.05)。Logistic回归分析显示治疗依从性差、家庭社会支持情况差、伴有精神病症状、miR-19b-3p(治疗前后)、重大生活事件均是影响抑郁症患者预后不良的危险因素(P<0.05)。ROC曲线分析显示,治疗前血清miR-19b-3p水平预测抑郁症患者预后不良的曲线下面积(area under curve,AUC)、敏感度、特异度、最佳截断点分别为0.739、78.56%、64.28%、11.25。治疗后血清miR-19b-3p水平预测抑郁症患者预后不良的AUC、敏感度、特异度、最佳截断点分别为0.756、80.75%、60.15%、7.14。结论血清miR-19b-3p在抑郁症患者体内呈异常高表达,与抑郁症患者预后不良具有一定的关系,可用于抑郁症患者预后的预测。 Objective To explore the predictive value of microRNA-19b-3p(miR-19b-3p)evaluated by Logistic regression combined with ROC curve in the treatment of depression.Methods A total of 112 patients with depression admitted to our hospital were selected as the research subjects(case group),and 60 healthy volunteers who presented to our hospital for physical examination during the same period were selected as the control group.The expression levels of peripheral serum miR-19b-3p in all subjects were detected by real-time quantitative polymerase chain reaction(PCR),and the expression levels of serum miR-19b-3p in the two groups before treatment and changes in the expression levels of serum miR-19b-3p in the case group before and after treatment were compared.According to the therapeutic effect,the case group was subdivided into a good prognosis group and a poor prognosis group,and the serum miR-19b-3p expression levels and other factors were compared between the good prognosis group and the poor prognosis group.Logistic regression analysis was used to identify the risk factors for poor prognosis in patients with depression,and receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of serum miR-19b-3p expression levels before and after treatment for poor prognosis in patients with depression.Results Before treatment,the serum miR-19b-3p level in the case group was higher than that in the control group(P<0.05),and after treatment,the serum miR-19b-3p level in the case group was lower than that before treatment(P<0.05).Family history of depression(positive),poor treatment compliance,poor family social support,poor relationship with neighbors,the proportion of major life events,the number of episodes,and serum miR-19b-3p levels before and after treatment were higher in the poor prognosis group than in the good prognosis group(P<0.05).Logistic regression analysis showed that poor treatment compliance,poor family social support,associated psychotic symptoms,miR-19b-3p(before and after treatment),a
