Analysis of influencing factors of prognosis of coronary heart disease

摘要 目的探讨凝血酶原片段1+2、D-二聚体、Gensini评分与冠心病预后的关系。方法选取江西省新余第四医院2017年1月~2018年2月收治的114例冠心病患者作为观察对象,依据冠心病预后不同分为不良心血管事件(MACE)组(34例)和非MACE组(80例)。分析冠心病预后的影响因素,采用COX模型多因素分析MACE的危险因素。结果非MACE组患者的D-二聚体、凝血酶原片段1+2均低于MACE组,Gensini评分高于MACE组,差异均有统计学意义(P<0.05)。两组患者的男性比例、高血压比例、左心室射血分数比较,差异无统计学意义(P>0.05)。建立COX回归模型,以出现MACE事件作为因变量,校正多因素COX回归分析结果显示,D-二聚体、凝血酶原片段1+2、Gensini评分是诱发MACE事件的独立预测因素,差异有统计学意义(P<0.05)。结论凝血酶原片段1+2、D-二聚体、Gensini评分是冠心病的独立预测因素,加强监测可以为指导临床治疗提供可靠的理论依据。 Objective To investigate the relationship between prothrombin fragment 1+2,D-dimer,Gensini score and prognosis of coronary heart disease(CHD).Methods A total of 114 patients with CHD admitted to our hospital from January 2017 to February 2018 were enrolled.According to the different prognosis of CHD,they were divided into the major adverse cardiovascular events(MACE)group(n=34)and the non-MACE group(n=80).The influencing factors of prognosis of CHD were analyzed.The risk factors of MACE were analyzed using COX model.Results The D-dimer and prothrombin fragment 1+2 in the non-MACE group were lower than those in the MACE group,the Gensini score in the non-MACE group was higher than that in the MACE group,with statistical significance(P<0.05).There was no significant difference in male ratio,hypertension ratio,or left ventricular ejection fraction between the two groups(P>0.05).A COX regression model was established.The occurrence of MACE was set as a dependent variable.The corrected multivariate COX regression analysis indicated D-dimer,prothrombin fragment 1+2,and Gensini score were independent predictors of MACE events,with statistical significance(P<0.05).Conclusion Prothrombin fragment 1+2,D-dimer and Gensini score are independent predictors of CHD.Strengthening monitoring can provide a reliable theoretical basis in the guidance of clinical treatment.
