摘要
目的采用Meta分析方法评价富血小板血浆(PRP)治疗重度烧伤的有效性,为临床用药治疗提供科学依据。方法计算机检索EMBase、PubMed、中国生物医学文献数据库、中国知网、维普中文科技期刊全文数据库、万方数据库、Cochrane数据库临床对照试验资料,同时筛查纳入的文献,纳入PRP治疗重度烧伤的随机对照试验(各数据库检索时间均从创建至2019年4月)。由2名专业人员独立提取资料进行核对、评价文献质量,采用Cochraneχ^2检验分析数据。结果本研究共纳入6篇文献。Meta分析结果显示,有效性方面,治疗组总有效率[RR=1.17,95%CI(1.08,1.26),P<0.0 001]、创面愈合时间[RR=-4.98,95%CI(-7.60,-2.36),P=0.0 002]、瘢痕指数[RR=-1.70,95%CI(-2.09,-1.30),P<0.00 001]以及视觉模拟评分法(VAS)评分[RR=-2.54,95%CI(-3.73,-1.34),P<0.00 001],治疗组与对照组相比,差异均有统计学意义(P值均小于0.05)。结论PRP在治疗重度烧伤方面较优秀,但此结论仍需更高质量临床随机对照试验予以证实。
Objective To evaluate the efficacy of platelet-rich plasma(PRP)in the treatment of severe burns by Meta-analysis method,and to provide scientific basis for clinical medication.Methods The data of clinical controlled trials such as EMBase,PubMed,CBM,CNKI,VIP,Wanfang and Cochrane were retrieved by computer.Meanwhile,the references of the literatures were screened and included in the randomized controlled trials of PRP in the treatment of severe burns(the retrieval time of all databases ranged from creation to April 2019).Data were independently extracted by two evaluators to check and evaluate the quality of literature.Cochrane chi-square test was used to analyze the data.Results Six articles were included in this study.Meta-analysis showed that the total effective rate of platelet-rich plasma treatment group[RR=1.17,95%CI(1.08,1.26),P<0.0 001],wound healing time[RR=-4.98,95%CI(-7.60,-2.36),P=0.0 002],scar index[RR=-1.70,95%CI(-2.09,-1.30),P<0.00 001]and visual analogue scale(VAS)score[RR=-2.54,95%CI(-3.73,-1.34),P<0.00 001],there were significant differences between the treatment group and the control group(P<0.05).Conclusion Based on the existing clinical evidence,PRP is superior in the treatment of severe burns,but this conclusion still needs to be confirmed by a higher quality clinical randomized controlled trial.
