摘要
目的:探讨弥散加权成像(DWI)及动态增强磁共振成像(MRI)对腮腺多形性腺瘤及腮腺Warthin's瘤的鉴别诊断价值。方法:收集经病理证实并术前行MRI检查的21例多形性腺瘤及20例Warthin's瘤患者的临床和影像学资料,分析比较两种肿瘤在临床特征、常规MRI信号特征、表观弥散系数最小值(ADCmin)及动态增强时间-信号强度曲线(TIC)方面的差异,评价ADCmin值及TIC在两种肿瘤鉴别诊断中的价值,并应用接受者操作特性(ROC)曲线确定两种肿瘤ADCmin的临界值。结果:两组资料在年龄、性别、形态、边界、深叶受累、T1WI信号、T2WI信号、信号均匀度及囊变方面差异均无统计学意义,最大径、TIC类型比较差异有统计学意义(t=-2.324,x^2=16.622;P<0.05);腮腺多形性腺瘤ADCmin值[(1.5±0.28)×10^-3 mm^2/s]显著高于Warthin's瘤ADCmin值[(0.91±0.16)×10^-3 mm^2/s],差异有统计学意义(t=8.565,P<0.05),当ADC以1.13×10^-3 mm^2/s作为临界值时,鉴别诊断多形性腺瘤及Warthin's瘤的敏感度及特异度分别为95.2%和90.9%。结论:在常规MRI基础上应用DWI及动态增强MRI,能为多形性腺瘤和Warthin's瘤的鉴别诊断提供更多依据,并提高其诊断准确性。
Objective:To explore the value of diffusion weighted imaging(DWI)and dynamic contrast-enhanced magnetic resonance imaging(MRI)in differential diagnosis of pleomorphic adenoma of parotid gland and Warthin's tumor.Methods:The clinical date and iconographical data of 21 patients(21 lesions)with pleomorphic adenoma of parotid gland and 20 patients with Warthin's tumor who were confirmed by pathology and were underwent MRI examination were collected.The differences of clinical characteristics,signal characteristics of routine MRI,the minimum value of apparent diffusion coefficient(ADCmin)and time-signal intensity curve(TIC)of dynamic enhancement were analyzed and compared.The values of ADC min value and TIC in differential diagnosis of the two tumors were evaluated,and the critical values of ADC min of two tumors were determined by the receiver operating characteristic(ROC)curve.Results:The differences of age,gender,shape,border,situation of deep lobe,T1WI signal,T2WI signal,homogeneousness and cystic change between the two groups were no significant(P>0.05).The differences of largest diameter and TIC type between the two groups were significant(t=-2.324,x^2=16.622,P<0.05).The value of ADC min of pleomorphic adenoma of parotid gland[(1.5±0.28)×10^-3 mm^2/s]was significantly higher than that of Warthin's tumor[(0.91±0.16)×10^-3 mm^2/s](t=8.565,P<0.05).The sensitivity and specificity of differential diagnosis of pleomorphic adenoma and Warthin's tumor were 95.2%and 90.9%respectively when 1.13×10^-3mm^2/s was used as critical value of ADC.Conclusion:On the basis of routine MRI,the application of DWI and dynamic contrast-enhanced MRI can provide more references for differential diagnosis of pleomorphic adenoma of parotid gland and Warthin's tumor and can improve the diagnostic accuracy for them.
