摘要
目的探讨阿尔茨海默病(AD)应用补肾化痰祛瘀方联合盐酸多奈哌齐治疗的临床效果。方法选取我院2015年7月—2017年7月收治的94例AD病人,采用随机数字表法均分为两组。对照组(47例)予以盐酸多奈哌齐治疗,观察组(47例)在对照组基础上加用补肾化痰祛瘀方治疗。连续治疗6个月后,比较两组临床疗效,治疗前后认知功能相关量表[简易精神状态量表(MMSE)、AD评定量表认知分量表(ADAS-cog)]评分、脑神经递质[多巴胺(DA)、去甲肾上腺素(NE)、5-羟色胺(5-HT)]水平及生活质量相关量表[日常生活能力量表(ADL)、老年抑郁量表(GDS)]评分变化。结果治疗6个月后,观察组总有效率为93.6%,较对照组(76.6%)明显升高(P<0.05)。与治疗前比较,两组治疗后MMSE评分均增高(P<0.05),ADAS-cog评分均减低(P<0.05);观察组改善较对照组差异有统计学意义(P<0.05)。两组治疗后血浆DA、NE、5-HT浓度较治疗前均增加(P<0.05),且观察组上升与较对照组比较差异有统计学意义(P<0.05)。与治疗前对比,两组治疗后ADL、GDS评分均减少(P<0.05);且观察组下降更优(P<0.05)。结论AD应用补肾化痰祛瘀方联合盐酸多萘哌齐治疗能有效改善病人症状,调控脑神经递质表达水平,提高认知功能和生活质量,延缓病情进展,疗效显著。
Objective To observe the clincical influence of Bushen Huatan Quyu Recipe combined with donepezil hydrochloride on cognitinve function of patients with Alzheimer′s Disease (AD). Methods Ninety-Four patients with AD were divided into control group and observation group,47 cases in each group.The patients in control group was treated with donepezil hydrochloride,and the patients in observation group was treated with Bushen Huatan Quyu Recipe combined with donepezil hydrochloride.After treatment for6 months,the clinical efficacy,cognitive function related scale [mini-mental state examination (MMSE),AD assessment scale (ADAS-cog)] scores,brain neurotransmitter [dopamine (DA),norepinephrine (NE),5-hydroxytryptamine (5-HT)] and life quality related scale [activities of daily living (ADL),geriatric depression scale (GDS)]scores were compared between the two groups. Results After6 months,the clinical efficacy of the observation group was 93.6% higher than that of the control group (93.6% vs 76.6%, P <0.05).After treatment,the MMSE scores of two groups were higher,and the observation group was significantly higher than in the control group ( P <0.05),the ADAS-cog scores of two groups were lower,and the observation group was significantly lower than in the control group ( P <0.05).After treatment,DA,NE,5-HT of two groups were increased ( P < 0.05),and that in the observation group was higher than in the control group ( P <0.05).After treatment,ADL and GDS scores of two groups were decreased ( P <0.05),and that in the observation group was lower than in the control group ( P <0.05). Conclusion Bushen Huatan Quyu Recipe combined with donapixil hydrochloride in the treatment of AD patients could effectively improve the symptoms of patients,regulate the expression of brain neurotransmitters,improve the cognitive function and quality of life,and postpone the progression of disease.
