摘要
目的探讨中西医结合加速康复外科(ERAS)策略在PPH手术治疗混合痔患者围手术期的应用效果。方法将在临沂市中心医院肛肠科住院并行PPH手术治疗的160例混合痔患者随机分为加速康复组和中西医结合加速康复组,每组各80例。比较两组术后不同时间点手术切口视觉模拟法(VAS)疼痛评分、总住院费用、住院时间、术后并发症和患者满意度评分情况。结果中西医结合加速康复组术后不同时间点手术切口(VAS)疼痛评分均低于加速康复组(均P<0.05);前者住院时间更短,满意度评分更高,术后尿潴留、排便困难、肛门坠胀发生率更低,差异均有统计学意义(均P<0.05);两组总住院费用比较差异无统计学意义(P>0.05)。结论中西医结合加速康复对行PPH手术的混合痔患者围手术期干预能够促进患者更快康复,且在术后手术切口(VAS)疼痛评分、术后并发症和满意度上优于单纯加速康复方案,安全性良好。
Objectives To investigate the effect of enhanced recovery after surgery (ERAS) combined with integrated traditional Chinese and western medicine in perioperative care for patients with mixed hemorrhoids receiving procedure for prolapse and hemorrhoids (PPH). Methods One hundred and sixty patients with mixed hemorrhoids treated with PPH at the Department of Anal and Intestinal Surgery, Linyi Central Hospital, were randomly assigned to ERAS group and Combination group (ERAS com bined with integrated traditional Chinese and western medicine) with 80 patients in each group. Visual analog scale (VAS) pain score at different time after surgery, hospitalization expenses, length of hospitalization, postoperative implications, and patients’sat isfaction were compared between the two groups. Results VAS pain scores significantly decreased in the Combination group com pared with those in the ERAS group (P < 0.05). Patients in the Combination group had significantly shorter length of hospitaliza tion, better patiens’satisfaction and lower incidence of urine retention, difficult defecation and postoperative anal bulge (P < 0.05). The hospitalization expenses did not differ between the groups (P > 0.05). Conclusion ERAS combined with integrated traditional Chinese and western medicine as perioperative care for patients with mixed hemorrhoids receiving PPH can effectively promote patients’recovery. Compared with ERAS alone, it is also associated with fewer postoperative pain, less postoperative complications, better safety, and better patients’satisfaction.
