摘要
目的探讨局部晚期非小细胞肺癌容积弧形调强放疗与固定野动态调强放疗剂量学差异。方法收集2018年1月至2019年2月于本院肿瘤科经影像学和病理学确诊为局部晚期非小细胞肺癌患者60例,采取固定野动态调强放射治疗和容积调强旋转放疗技术治疗。比较两种计划的DVH与等剂量分布,靶区评估最大剂量、最小剂量、平均剂量及适形指数,危及器官评估指标为:脊髓最大剂量,心脏V30,食管V50、V60及平均剂量,全肺V5、V10、V20和V30。结果容积调强旋转放疗适形指数高于固定野调强放射治疗,差异具有统计学意义(P<0.05);计划靶区最大剂量、平均剂量、最小剂量、不均匀指数、适形指数方面,危及器官的脊髓最大剂量,心脏V30,食管V50,V60及平均剂量,全肺V5,V10,V20和V30对比,差异无统计学意义。结论局部晚期非小细胞肺癌容积弧形调强放疗与固定野动态调强放疗治疗,均能满足临床治疗需要,但容积调强旋转放疗适形度更好,可优先选择容积调强旋转放疗治疗方案。
Objective To investigate the dosimetric difference between volume-arc IMRT and fixed-field dynamic IMRT for locally advanced non-small cell lung cancer (NSCLC). Methods From January 2018 to February 2019, 60 patients with locally advanced non-small cell lung cancer diagnosed by radiology and pathology in the oncology department of our hospital were treated with fixed field intensity modulated radiotherapy and volume intensity modulated rotational radiotherapy. DVH and isodose distribution of the two schemes were compared. Maximum dose, minimum dose, average dose and conformity index of target area were evaluated. The organ-threatening indexes were maximal dose of spinal cord, V 30 of heart, V 50 of esophagus, V 60 and average dose, V 5 , V 10 , V 20 and V 30 of whole lung. Results The conformal index of volume-modulated rotational radiothera- py was higher than that of fixed field intensity-modulated radiotherapy (P<0.05). There was no difference in the maximum dose of spinal cord, heart V 30 , esophageal V 50 , V 60 and average dose of organ-threatening organs in planned target area, whole lung V 5 , V 10 , V 20 and V 30 (P<0.05). The difference was not statistically significant. Conclusion Both volume-modulated radiotherapy and fixed field dynamic intensity-modulated radiotherapy for locally advanced non-small cell lung cancer can meet the needs of clinical treatment, but volume-modulated rotational radiotherapy has better conformity, so volume-modulated rotational radiotherapy can be preferred.
