摘要
目的 研究地佐辛复合右美托咪定对食道癌术后镇痛效果。方法 选取2015年1月至2018年9月来本院进行食道癌手术的患者150例,采用随机数字表法分为A组、B组、C组,各50例。患者完成手术后,A组患者采用8μg/kg芬太尼注射液进行镇痛;B组患者采用1μg/kg右美托咪定+8μg/kg芬太尼注射液进行镇痛;C组患者采用0.6mg/kg地佐辛+1μg/kg右美托咪定+8μg/kg芬太尼注射液进行镇痛。观察三组患者术后2h、4h、8h、24h后的疼痛指数、Ramsay镇静评分及不良反应发生率。结果 C组患者VAS评分、Ramsay镇静评分均明显优于A、B组,差异均有统计学意义(均P<0.05);C组不良反应发生率28.0%,明显低于A、B组82.0%、74.0%,差异有统计学意义(P<0.05)。结论 地佐辛复合右美托咪定可有效减少食道癌手术患者术后疼痛指数、躁动指数及不良反应发生率,值得临床应用及推广。
Objective To study the postoperative analgesic effect of dezocine combined with dexmedetomidine on esophageal cancer. Methods A total of 150 patients who underwent esophageal cancer surgery in our hospital from January 2015 to September 2018 were enrolled. The patients were divided into group A, group B, and group C by the random number table method, and 50 patients in each group. After the operation completed, the patients in group A received analgesia with 8 μg/kg fentanyl injection;the patients in group B received analgesia with 1 μg/kg dexmedetomidine + 8 μg/kg fentanyl injection;the patients in group C received analgesia with 0.6 mg/kg dezocine + 1 μg/kg dexmedetomidine + 8 μg/kg fentanyl injection. Pain index, Ramsay sedation score, and adverse reaction rate were observed at 2 h, 4 h, 8 h and 24 h after the operation in the three groups. Results The results of the study showed that the VAS score and Ramsay sedation score in group C were significantly better than those in group A and B;the incidence of adverse reactions in group C (28.0%) was significantly lower than that in group A and B(82.0%, 74.0%). The differences of the results among the three groups were statistically significant (P<0.05). Conclusion Dezocine combined with dexmedetomidine can effectively reduce postoperative pain index, agitation index, and incidence of adverse reactions in patients with esophageal cancer surgery, which is worthy of clinical application and promotion.
