摘要
目的 观察康复新液联合曲安奈德治疗糜烂型口腔扁平苔藓(OLPe)的临床效果及其对血液流变学的影响。方法 将2017 年10 月至2018 年9 月在本院进行治疗的口腔扁平苔藓患者96例,按照随机数字表法分成对照组和研究组,各48例。对照组曲安奈德注射液治疗,研究组注射曲安奈德联合康复新液局部湿敷患处治疗。3 个疗程后,比较两组临床疗效、糜烂处愈合时间、疼痛指数(VAS 评分)、血清炎症因子水平和血液流变学指标的变化。记录并比较两组治疗过程中不良反应发生情况。结果 研究组总有效率为93.7%,明显优于对照组的72.9%(P<0.05),临床症状缓解程度也明显优于对照组(P< 0.05);研究组平均糜烂处愈合时间(5.31±2.03)d,明显短于对照组(9.74±3.25)d,差异具有统计学意义(P<0.05);两组炎症因子白介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)和白介素-10(IL-10)均显著下降(均P<0.05),且研究组治疗后各血清炎症因子均明显低于对照组(均P<0.05);研究组治疗后纤维蛋白原、血小板聚集率和血浆粘度明显低于治疗前(P<0.05),对照组无显著变化(均P> 0.05)。两组用药后均未发生明显的不良反应。结论 康复新液联合曲安奈德治疗OLPe 效果显著,患者疼痛感可以得到有效缓解,且有效改善患者血液流变学指标,值得临床推广。
Objective To observe the clinical effect Kangfuxin solution combined with triamcinolone acetonide for erosive oral lichen planus (OLPe) and its influence on blood rheology indicators. Methods 9 6 patients with erosive oral lichen planus treated in our hospital from October, 2017 to September, 2018 were randomly divided into a control group and a study group, 48 cases for each group. The control group were treated with injections of triamcinolone acetonide , while the study group were given Kangfuxin solution and triamcinolone acetonide. After three courses of treatment, the clinical effect, erosion area healing time, and Visual Analogue Scale (VAS) score were compared between the two groups. In addition, the serum inflammatory factors and blood rheology levels before and after the treatment in the two groups were observed. The adverse reactions were also recorded during the treatment. Results The total effective rate of the study group was 93.7%, and that of the control group 72.9%(P < 0.05). The clinical symptom relief degree was obviously better in the study group than in the control group (P < 0.05). The average erosion healing time of the study group was (5.31±2.03) d, that of the control group (9.74±3.25) d (P < 0.05). After the treatment, the interleukin-2(IL-2), tumor necrosis factor alpha (TNF-α), and interleukin-10(IL-10) decreased in both groups (all P < 0.05), and were lower in the study group than in the control group (all P < 0.05).The serum fibrinogen, platelet aggregation rate, and plasma viscosity were lower after than before the treatment in the study group (all P < 0.05), while the control group’s blood rheology levels between before and after the treatment showed no significant difference (all P > 0.05). There were no obvious adverse reactions after the treatment in these two groups. Conclusion Kangfuxin solution combined with triamcinolone acetonide can effectively improve the clinical effect and blood rheology indicators,relieve the patients’ pain, so it is worthy of clinical promotion
