经皮椎间孔入路脊柱内镜下髓核摘除入和椎间盘切除术术治疗L5/S1椎间盘突出症的效果对比 被引量:11

摘要 手术治疗能直接摘除椎间盘突出症(LDH)突出及变性的髓核,使患者疼痛得以缓解,但手术造成的创伤仍对患者术后恢复影响较大,故微创手术受到学术界广泛关注。经皮椎间孔入路脊柱内镜下髓核摘除术(PETD)为LDH的常用微创疗法,具有切口小、出血少、机体反应轻等优点。但受机体髂嵴阻挡、椎间孔较小等因素影响,L5/S1 LDH患者经PETD治疗时可出现穿刺困难等情况,使手术时间延长。而经皮内镜椎板间入路椎间盘切除术(PEID)可通过切开黄韧带等方式进入椎间孔,无需磨削上关节突,影响脊柱结构。基于此,本研究回顾性分析经PEID术治疗及经PETD术治疗的L5/S1 LDH患者各298例临床资料,以评估2种术式临床疗效。
