摘要
目的探讨氯普鲁卡因联合吗啡硬膜外麻醉在剖宫产术中的镇痛效果及其对产妇炎性因子的影响.方法选取蒙阴县妇幼保健院2016年3月-2018年5月收治的剖宫产产妇80例,采用计算机分组法分为对照组和试验组,各40例.对照组产妇实施吗啡硬膜外麻醉,试验组产妇实施氯普鲁卡因与吗啡联合硬膜外麻醉.比较两组产妇术后6、12 h视觉模拟量表评分法(VAS)评分,手术前后白介素6和白介素10水平,并观察两组产妇不良反应发生情况.结果试验组产妇术后6、12 h VAS评分低于对照组(P<0.05).术前两组产妇白介素6、白介素10水平比较,差异无统计学意义(P>0.05);术后试验组产妇白介素6、白介素10水平低于对照组(P<0.05).两组产妇均未发生严重不良反应.结论氯普鲁卡因联合吗啡硬膜外麻醉在剖宫产术中的镇痛效果较优,可降低产妇炎性因子水平,减轻炎性反应.
Objective To investigate the analgesic effect of chloroprocaine combined with morphine epidural in anesthesia cesarean section and its influence on inflammatory factors.Methods A total of 80 cases of cesarean section women were selected from March 2016 to May 2018 in Mengyin County Maternal and Child Health Hospital,which were divided into control group and experimental group according to computer grouping method,40 cases in each group.The control group was given morphine epidural anesthesia,while the experimental group was given chloroprocaine and morphine combined epidural anesthesia.The VAS scores of 6 and 12 hours after operation were compared between the two groups,and the levels of IL-6 and IL-10 before and after operation were compared between the two groups,and the incidence of adverse reactions were observed. Results The VAS score of puerpera in the experimental group was lower than control group at 6 and 12 hours after operation (P<0.05).There was no significant difference in the levels of IL-6 or IL-10 between the two groups before operation(P>0.05);the experimental group of IL-6 and IL-10 level was lower than that control group(P<0.05).No serious adverse reactions occurred in both groups.Conclusion The analgesic effect of chloroprocaine combined with morphine epidural in anesthesia cesarean section is better,it can reduce the level of inflammatory factors and inflammatory reaction index of parturients.
