血清B型钠尿肽联合Copeptin在慢性心力衰竭患者预后评价中的价值 被引量:21

摘要 目的探究血清B型钠尿肽(BNP)联合Copeptin应用在慢性心力衰竭(chronic heart failure,CHF)患者预后评价中的价值。方法选取2017年6-9月在广东药科大学附属第一医院心内科住院的56例慢性心力衰竭患者,检测并记录患者入院时血清Copeptin和BNP水平,出院后进行定期随访,采用ROC曲线评估单独及联合应用Copeptin和BNP与慢性心力衰竭患者短期预后的关系,通过Kaplan-Meier法进一步探究生物标志物升高个数与慢性心力衰竭患者短期不良终点事件发生率的相关性。结果ROC结果显示Copeptin与BNP曲线下面积分别为0.68(95%CI:0.50~0.85)、0.70(95%CI:0.55~0.86)。两者联合应用时面积为0.79(95%CI:0.66~0.92)。Kaplan-Meier曲线结果表明BNP与Copeptin均升高的患者发生不良终点事件的频率明显高于单个或无生物标志物升高的患者(P<0.05)。多因素分析结果表明Copeptin(RR=1.406,P=0.007)、BNP(RR=3.865,P=0.047)。结论BNP与Copeptin是慢性心力衰竭患者短期不良终点事件的预测因子,血清BNP与Copeptin联合应用对慢性心力衰竭患者短期预后预测的准确度高于单个生物标志物单独应用。 Objective To explore the value of application of serum BNP and Copeptin in prognosis evaluation of patients with chronic heart failure.Methods 56 patients with chronic heart failure who were hospitalized in the First Hospital Affiliated to Guangdong Pharmaceutical University from June to September,2017 were selected.In regular follow-ups,patients′ Copeptin and BNP levels were measured and recorded;the relationship between the separated and combined application of serum BNP and Copeptin and the short-term prognosis of patients with chronic heart failure was evaluated by ROC curve;explore the relevance between the increase of biomarkers and incidence of short-term adverse endpoint events of patients with chronic heart failure by Kaplan-Meier estimator.Results ROC results showed that the area under the curve of Copeptin and BNP was respectively 0.68(95%CI: 0.50 ~ 0.85)and 0.70(95%CI:0.55 ~ 0.86).When Copeptin and BNP are both applied,the area was 0.79(95%CI: 0.66 ~ 0.92).Kaplan-Meier curve results showed that patients with elevated BNP and Copeptin had significantly higher frequency of adverse endpoint events than patients with single or no increased biomarkers(P<0.05).Multivariate analysis showed that Copeptin(RR=1.406,P=0.007),BNP(RR=3.865,P=0.047).Conclusions BNP and Copeptin are predictors of short-term adverse endpoint events of patients with chronic heart failure.The combined application of serum BNP and Copeptin is more accurate than single biomarker in prediction in shortterm prognosis of patients with chronic heart failure.
