摘要
目的提高对结核性“树芽征”CT影像特征的认识水平,明确CT所见的结核性“树芽征”相对应的病理意义。方法对照分析7例经手术治疗的继发性肺结核患者的CT影像表现及相对应的病理结果,复习文献。结果结核性“树芽征”的“树状”部分对应于小叶内炎性细支气管,而“芽状”部分对应充满了炎性物质的比相应的细支气管粗大的肺泡管,“树芽状”病变包括腔内干酪或支气管内、支气管周围干酪样病变、炎性细胞和碎屑、纤维化和肉芽肿形成的混合物。结论结核性“树芽征”代表肺结核支气管腔内播散过程中的特殊病理过程,在临床工作中可以为肺结核诊治提供证据。
Objective To improve the understanding of CT features of tuberculous"tree-in-bud-pattern"and clarify the pathological significance of"tree-in-bud-pattern"of tuberculosis.Methods CT findings and pathological results of 7 patients with secondary pulmonary tuberculosis treated by operation were analyzed and the literature was reviewed.Results The"tree"(stem)part of tuberculous"tree-in-bud-pattern"corresponds to the inflammation bronchioles in the lobules,while the"buds"part corresponds to the bronchioles with larger alveolar tubes filled with inflammatory substances than the corresponding bronchioles.The"tree-in-bud-pattern"lesions include intraluminal cheese or intrabronchial or peribronchial cheese-like lesions,inflammatory cells and debris.A mixture of fibrosis and granuloma formation.Conclusion Tuberculous"tree-in-bud-pattern"represents a special pathological process in the process of bronchial dissemination of tuberculosis,which can provide evidence for the diagnosis and treatment of tuberculosis in clinical work.
