超声征象对乳腺癌生物标记物HER2和ER、PR、Ki-67不同表达水平的诊断意义 被引量:4

摘要 目的:探讨乳腺癌生物标记物HER2和ER、PR不同表达水平组间的乳腺癌超声征象的差异。方法:收集我院诊治的乳腺癌患者62例70个病灶,行免疫组化检测乳腺癌组织中乳腺癌生物标记物HER2和ER、PR、Ki-67的表达,根据上述生物标记物的表达水平进行分组,观察各组的超声征象包括后方回声、血流紊乱、边缘、内部强化方式、肿块形状及肿块大小等。结果:与HER2阴性组比较,HER2阳性组的边缘不整齐、分叶状和内部低回声等超声征象方面比较差异具有统计学意义(P<0.05);与ER和PR均阴性组比较,ER和PR均阳性组的肿块内部血流等级、内部强化方式和后方回声等方面比较差异具有统计学意义(P<0.05);与Ki-67低表达组比较,Ki-67高表达组纵横径比、内部强化方式和血流等级等方面比较差异具有统计学意义(P<0.05);其他征象各组间比较差异均无统计学意义(P>0.05)。结论:乳腺癌生物标记物的表达水平可呈现不同的超声征象特征,通过超声检测技术可在一定程度上反应肿瘤的恶性生物学行为特征和生物进展过程,对指导临床诊治具有重要的临床意义。 Objective:To explore the difference of ultrasonographic features of breast cancer between different expression levels of HER2, ER and PR group.Methods:62 cases of 70 breast cancer lesions were collected, and immunohistochemical detection was used to detect breast cancer tissue biomarkers of ER, PR, Ki - 67 and HER2. According to the expression level of biomarkers, groups of ultrasonic signs including rear echo, blood disorders, edge, internal reinforcement method, lump shape and the size of the lump were observed.Results:Compared with the HER2 negative group, there were statistically significant differences in the edge irregularity, lobulation and internal hypoechoic features of the HER2 positive group ( P <0.05). Compared with the ER and PR negative groups, there were statistically significant differences in tumor internal blood flow grade, internal enhancement pattern and posterior echo between the ER and PR positive groups ( P <0.05). Compared with the low-expression group of ki-67, there were statistically significant differences in the aspect of aspect ratio, internal enhancement pattern and blood flow level in the high-expression group of ki-67 ( P <0.05). There was no statistically significant difference between the other groups ( P >0.05).Conclusion:The expression level of breast cancer biomarkers can present different ultrasonic features. The ultrasonic detection technology can reflect the malignant biological behavior characteristics and biological progression of tumors to a certain extent, which is of great clinical significance in guiding clinical diagnosis and treatment.
