摘要
目的呼吸内科患者病原菌的分布及耐药性,为临床合理抗感染治疗提供依据。方法选择2017年1月至2018年12月呼吸内科住院患者送检的全部标本中分离的病原菌,分析病原菌分布情况、构成比及耐药率,并统计多重耐药菌的检出情况。结果2017年1月至2018年12月共分离出病原菌489株,其中革兰阴性菌333株(68.10%),革兰阳性菌34株(6.95%),真菌122株(24.95%)。革兰阴性菌主要为铜绿假单胞菌(PA)(33.13%)、肺炎克雷伯杆菌(13.70%)、大肠埃希菌(7.57%)、鲍曼不动杆菌(AB)(6.13%)。革兰阴性菌中肺炎克雷伯菌、大肠埃希菌对碳青霉烯类、加酶复合制剂耐药率低,PA及AB对碳青霉烯类耐药率较高。革兰阳性菌对万古霉素、利奈唑胺耐药率较低。结论今后临床工作中应根据药敏结果规范应用抗菌药物,加强多重耐药菌的管理,降低病原菌耐药性。
Objective To analyze the distribution and drug resistance of pathogenic bacteria of patients in respiratory medicine department, and to provide basis for rational anti-infective treatment in clinic. Methods Pathogenic bacteria isolated from all specimens of hospitalized patients in respiratory medicine department from January 2017 to December 2018 were selected. The distribution, composition and drug resistance of pathogenic bacteria were analyzed, and the detection of multiple resistant bacteria was counted. Results A total of 489 strains of pathogenic bacteria were isolated from January 2017 to December 2018, of which there were 333 strains (68.10%) Gram-negative bacteria, 34 strains (6.95%) Gram-positive bacteria and 122 strains (24.95%) fungi. There were mainly Pseudomonas aeruginosa (PA)(33.13%), Klebsiella pneumoniae (13.70%), Escherichia coli (7.57%) and Acinetobacter baumannii (AB)(6.13%) in Gram-negative bacteria Among Gram-negative bacteria, Klebsiella pneumoniae and Escherichia coli showed low resistance to carbapenems and compound preparation with enzyme, while PA and AB showed high resistance to carbapenems. The resistance rates of the Gram-positive bacteria to vancomycin and linezolid were low. Conclusion It is suggesting that antibiotics should be normativelyapplied according to the results of drug sensitivity results in the future clinical work, the management of multiple resistant bacteria should be strengthened to reduce the resistance of pathogenic bacteria.
