糖尿病肾脏疾病血液透析患者与非糖尿病肾脏疾病血液透析患者人体成分测定分析 被引量:12

摘要 目的了解糖尿病肾脏疾病(DKD)血液透析患者与非糖尿病肾脏疾病(non-DKD)血液透析患者人体成分的差异,对不同血液透析患者身体水分容量状况及相位角进行评价。方法选取深圳市中医院血液透析患者85例,按其基础肾脏病分为DKD血液透析患者组(31例),non-DKD血液透析患者组(54例)。使用InBodyS10人体成分分析仪,测量两组患者细胞外水分比率、相位角等人体成分参数,并进行组间比较。结果DKD血液透析患者与non-DKD血液透析患者相比,患者的性别、年龄、身高、超滤量均未见显著差异(P>0.05)。DKD组平均血压及透析间期体质量、干体质量显著高于non-DKD组,但各节段相位角均低于non-DKD组(P<0.05)。此外,DKD组细胞外水分含量以及全身和各节段细胞外水分均高于non-DKD组(P<0.05)。结论相比较于non-DKD血液透析患者,DKD血液透析患者身体水分分布异常情况及高血压状况更为严重,同时DKD血液透析患者营养不良风险显著增加。 Objective To understand the difference of body composition between hemodialysis patients with diabetic kidney diseases (DKD) and hemodialysis ones with non-DKD, and evaluate the balance of body fluid and phase angle in different hemodialysis patients. Methods A total of 85 hemodialysis patients in Shenzhen Municipal TCM Hospital were selected, and divided into DKD- hemodialysis patient group (31 cases) and non-DKD hemodialysis patient group (54 cases) based on their basic kidney diseases. The body composition analyzer was used to measure the body composition parameters such as extracellular fluid ratio and phase angle, and to conduct comparison between the two groups. Results In comparison between DKD-hemodialysis patients and non-DKD-hemodialysis ones , there was no significant difference in gender, age, height and ultrafiltration volume ( P >0.05). Mean blood pressure, and interdialytic body weight and dry body weight with the DKD group were significantly higher than those with the non-DKD group, but phase angles of each segment with the DKD group were lower than that with the non-DKD group ( P <0.05). In addition, extracellular fluid content and fluid contents in the whole body and each segment with the DKD group were higher than those with the non- DKD group( P < 0.05 ). Conclusions Compared to the hemodialysis patients with non-DKD, the hemodialysis ones with DKD have more severe body fluid distribution abnormality and hypertension, and significantly increased risk for malnutrition.
