纺锤体与动粒相关蛋白-1在肝细胞癌中的预后分析

摘要 目的探讨纺锤体与动粒相关蛋白-1(SKA1)在肝细胞癌(HCC)中的表达以及其对患者预后的影响。方法收集88对HCC癌组织和对应的癌旁组织,采用免疫组化的方法对这些组织中SKA1的表达水平进行了检测,分析其表达水平与HCC患者临床病理参数的关系,并采用Kaplan-Meier法、单因素和多因素Cox比例风险回归模型分析SKA1的表达水平对HCC患者预后的影响。结果与癌旁组织相比,SKA1在HCC癌组织中显著高表达(P<0.001)。SKA1的表达水平与瘤体大小密切相关(P<0.002)。Kaplan-Meier法表明SKA1高表达组的5年总生存率和无瘤生存率分别为27.03%、35.14%,而低表达组的5年总生存率和无瘤生存率分别为60.78%、50.98%(P<0.05)。亚组分析表明,高表达的SKA1能预测血浆高水平谷丙转氨酶(ALT)患者的总生存率和无瘤生存率(P<0.05),而在血浆低水平ALT患者中差异无统计学意义(P>0.05)。多因素Cox比例风险回归模型表明SKA1是HCC总生存率的独立危险因素(HR=2.879,P=0.001),但不是无瘤生存率的独立危险因素(HR=1.718,P=0.075)。结论SKA1参与了HCC的发生发展,可以作为HCC患者总生存率的预后指标。 Objective To explore the expression and its prognostic value of spindle and kinetochore-associated complex subunit 1 (SKA1) in hepatocellular carcinoma (HCC). Methods SKA1 expression was evaluated by immunohistochemistry method in 88 pairs of HCC and adjacent tumor-free tissues.The relation between its expression and clinicopathological parameters was also analyzed.The prognostic value of SKA1 in HCC was explored by using Kaplan-Meier survival curves,univariate and multivariate cox proportional hazards regression analysis. Results Compared to adjacent tissues,SKA1 was significantly higher in HCC tissues ( P <0.001).SKA1 expression was positively correlated with tumor size ( P =0.002).Kaplan-Meier survival curves revealed that 5-year overall survival rate and disease-free survival rate for patients with high SKA1 expression were 27.03% and 35.14%,respectively;while for low SKA1 expression,it was 60.78% and 50.98%,respectively ( P <0.05).Moreover,subgroup analysis showed that high SKA1 expression predicted overall survival and disease-free survival only for high alanine transaminase (ALT) level ( P <0.05),whereas there was no statistically significant difference in patients with low plasma ALT ( P >0.05).Multivariate analysis showed that SKA1 overexpression was an independent indicator for overall survival ( HR =2.879, P =0.001),but not for disease-free survival ( HR =1.718, P =0.075). Conclusion SKA1 participates in the development of HCC and serves as an independent predictor of overall survival for patients with HCC.
