摘要
目的:探讨3.0T高分辨核磁共振成像(HR-MRI)对大脑中动脉(MCA)粥样硬化的诊断价值。方法:回顾性选取在医院治疗的120例MCA粥样硬化患者资料,根据临床症状性MCA将其分为有症状组(56例)和无症状组(64例)。两组均行HRMRI和数字减影血管造影(DSA)检查,同时与DSA的MCA M1狭窄程度进行比较。结果:HR-MRI与DSA检查判断MCA大脑中动脉M1狭窄程度差异有统计学意义(Kappa=0.539,P<0.05);HR-MRI诊断MCA的M1中重度狭窄(狭窄≥50%)的灵敏度、特异度、阳性预测值和阴性预测值分别为100.00%、79.17%、95.05%和100.00%;有症状组和无症状组MCA血管最狭窄处血管面积、管腔面积和管壁面积比较差异无统计学意义;有症状组MCA患者斑块大小和斑块负荷分别为(6.24±1.12)mm2和(0.43±0.10)mm2,明显高于无症状组MCA患者,其差异有统计学意义(t=9.469,t=7.495;P<0.05)。结论:3.0T HRMRI在MCA粥样硬化诊断中有一定价值,可对狭窄管壁及斑块进行评估。
Objective: To explore the diagnostic value of 3.0T high resolution magnetic resonance imaging (HR-MRI) in middle cerebral artery (MCA) atherosclerosis. Methods: The data of 120 patients with MCA atherosclerosis who received treatment in hospital were retrospectively selected and they divided into symptomatic group (56 cases) and asymptomatic group (64 cases) according to clinical symptoms of MCA. HR-MRI and digital subtraction angiography (DSA) were performed in both groups, at the same time, the stenosis degree of MCA M1 of HR-MRI was compared with that of DSA. Results: There was significant difference between HR-MRI and DSA in judging the stenosis degree of MCA M1 (Kappa=0.539, P<0.05). The sensitivity, specificity, positive predictive value and negative predictive value of HRMRI in diagnosing middle and severe stenosis (stenosis ≥50%) of MCA M1were 100.00%, 79.17%, 95.05% and 100.00%, respectively. The differences of the area of blood vessel, the area of lumen and the area of tube wall at the most stenosis site of MCA between the symptomatic group and the asymptomatic group were no significant. The plaque size and plaque load of symptomatic group (6.24+1.12 mm2 and 0.43+0.10 mm2, respectively) were significantly higher than those of asymptomatic group (t=9.469, t=7.495, P<0.05). Conclusion: 3.0T HR-MRI has a certain value in diagnosing MCA atherosclerosis and it can be used to assess the tube wall of stenosis and plaque.
