性激素结合蛋白、胰岛素受体底物、葡萄糖转运蛋白表达及在妊娠期糖尿病发病过程中的作用分析 被引量:13

摘要 目的探讨妊娠期糖尿病孕妇胎盘组织中性激素结合球蛋白(SHBG)、胰岛素受体底物(IRS)、葡萄糖转运蛋白(GLUT)的表达及在妊娠期糖尿病发病过程中的作用,为临床诊治提供参考依据。方法收集2017年1月-2018年6月在南开医院产科定期产检,择期行剖宫产手术的单胎足月孕妇,其中妊娠期糖尿病孕妇120例为妊娠期糖尿病组,糖代谢正常孕妇120例为正常对照组。应用实时PCR(qPCR)和Western blot技术检测两组孕妇胎盘组织中SHBG、IRS-1、IRS-2、PI3K-p85β、GLUT1、GLUT3、GLUT4 mRNA和蛋白的表达水平,并分析各指标间的相关性。结果妊娠期糖尿病组孕妇胎盘组织中SHBG、IRS-2、PI3K-p85β、GLUT3、GLUT4 mRNA表达水平明显低于正常对照组,差异有统计学意义(均P<0.05);两组孕妇胎盘组织中IRS-1、GLUT1 mRNA表达水平差异无统计学意义(均P>0.05)。妊娠期糖尿病组孕妇胎盘组织中SHBG、IRS-1、GLUT3、GLUT4蛋白表达水平明显低于正常对照组,差异有统计学意义(均P<0.05);两组孕妇胎盘组织中IRS-2、PI3K-p85β、GLUT1蛋白表达水平差异无统计学意义(均P>0.05)。Pearson分析显示,SHBG与IRS-2、PI3K-p85β、GLUT3、GLUT4 mRNA表达呈正相关;IRS-2与GLUT1、GLUT4 mRNA表达呈正相关;IRS-1与PI3K-p85β、GLUT3 mRNA表达呈负相关。SHBG与IRS-2、GLUT3、GLUT4蛋白表达呈正相关;IRS-2与GLUT1、GLUT4蛋白表达呈正相关;IRS-1与GLUT3蛋白表达呈负相关。结论妊娠期糖尿病孕妇胎盘组织中SHBG、IRS及GLUT表达异常;SHBG浓度降低可能通过影响胰岛素信号传导通路导致胎盘葡萄糖利用障碍,最终引发妊娠期糖尿病。 Objective To explore the expressions of sex hormone-binding globulin ( SHBG),insulin receptor substrate ( IRS),glu- cose transporter ( GLUT) and the roles in pathogenesis of gestational diabetes mellitus ( GDM),provide a reference basis for clinical diagno- sis and treatment of GDM. Methods The singleton and full-term pregnant women receiving regular prenatal examination and selective ce- sarean section in Nankai Hospital from January 2017 to June 2018 were collected,then they were divided into GDM group ( 120 pregnant women with GDM) and control group ( 120 pregnant women with normal glucose metabolism). Real-time PCR and Western blot were used to detect the expression levels of SHBG,IRS-1,IRS-2,PI3K-p85β,GLUT1,GLUT3,and GLUT4 mRNA and protein in placental tissues of the two groups,the correlations between these indexes were analyzed. Results The expression levels of SHBG,IRS-2,PI3K-p85β, GLUT3,and GLUT4 mRNA in placental tissue in GDM group were statistically significantly lower than those in control group ( P<0. 05). There was no statistically significant difference in the expression levels of IRS-1 and GLUT1 mRNA in placental tissue between the two groups ( P>0. 05). The expression levels of SHBG,IRS-1,GLUT3,and GLUT4 protein in placental tissue in GDM group were statistical- ly significantly lower than those in control group ( P<0. 05). There was no statistically significant difference in the expression levels of IRS- 2,PI3K-p85β,and GLUT1 protein in placental tissue between the two groups ( P>0. 05). Pearson analysis showed that SHBG mRNA ex- pression was positively correlated with expressions of IRS-2,PI3K-p85β,GLUT3,GLUT4 mRNA,IRS-2 mRNA was positively correlated with expressions of GLUT1 and GLUT4 mRNA,IRS-1 mRNA was negatively with expressions of PI3K-p85β and GLUT3 mRNA,SHBG protein was positively correlated with expressions of IRS-2,GLUT3,and GLUT4 protein,IRS-2 protein was positively correlated with ex- pressions of GLUT1 and GLUT4 protein,IRS-1 protein was negatively correlated with GLUT3 prote
