摘要
目的探究老年2型糖尿病(T2DM)患者使用胰岛素泵的疗效。方法90例老年2型糖尿病患者,随机分为观察组与对照组,每组45例。观察组患者给予胰岛素泵治疗,对照组患者给予每日多次皮下注射胰岛素(MDI)治疗。比较两组患者治疗前后血糖水平及血糖达标时间、低血糖发生情况、胰岛素用量。结果治疗前,两组患者空腹血糖、餐后2h血糖水平比较差异无统计学意义(P>0.05)。治疗后,两组患者空腹血糖、餐后2h血糖水平均较本组治疗前下降,观察组患者下降程度更明显,差异具有统计学意义(P<0.05)。观察组患者血糖达标时间(4.58±0.52)d短于对照组的(7.89±1.26)d,胰岛素日用量(36.70±0.02)U/d少于对照组的(49.50±8.76)U/d,差异具有统计学意义(P<0.05)。观察组患者低血糖发生率低于对照组,但差异无统计学意义(P>0.05)。结论胰岛素泵治疗老年2型糖尿病被证明是安全有效的,胰岛素用量少,简单方便。
Objective To investigate the efficacy of insulin pump in elderly patients with type 2 diabetes mellitus (T2DM). Methods A total of 90 elderly patients with type 2 diabetes mellitus were randomly divided into observation group and control group, with 45 cases in each group. The observation group was treated with insulin pump, and control group was treated with multiple daily injections (MDI). Comparison were made on blood glucose before and after treatment, blood glucose standard-reaching time, occurrence of hypoglycemia and insulin dosage in two groups. Results Before treatment, both groups had no statistically significant difference in fasting blood glucose and 2 h postprandial blood glucose (P>0.05). After treatment, both groups had lower fasting blood glucose and 2 h postprandial blood glucose than those before treatment, and the observation group had more obvious decline. Their difference was statistically significant (P<0.05). The observation group had shorter blood glucose standard-reaching time as (4.58±0.52) d than (7.89±1.26) d in the control group, and less daily dosage of insulin as (36.70±0.02) U/d than (49.50±8.76) U/d in the control group. Their difference was statistically significant (P<0.05). The observation group had lower incidence of hypoglycemia than the control group, but the difference was not statistically significant (P>0.05). Conclusion Insulin pump treatment for elderly type 2 diabetes has proven to be safe and effective with less dosage of insulin and it is simple and convenient.
