摘要
目的观察针刺联合益气温阳汤治疗心肾阳虚型慢性心力衰竭的疗效及对血浆B型脑钠肽(BNP)和心功能的影响。方法将100例心肾阳虚型慢性心力衰竭患者随机分为两组,对照组50例给予益气温阳汤治疗,观察组50例给予针刺联合益气温阳汤治疗,两组均持续治疗4周。比较两组治疗后临床疗效,统计两组治疗前后中医证候积分、血浆BNP、心功能和生活质量评分。结果观察组总有效率为94.0%,显著优于对照组的76.0%(P<0.05);观察组和对照组治疗后中医证候积分、血浆BNP水平、左室舒张末期内径(LVEDd)、生活质量评分均较治疗前显著下降(P<0.05),且观察组低于对照组(P<0.05);观察组和对照组治疗后左室射血分数(LVEF)、心搏出量(SV)和心输出量(CO)均较治疗前显著上升(P<0.05),且观察组均高于对照组(P<0.05)。结论针刺联合益气温阳汤治疗心肾阳虚型慢性心力衰竭,疗效确切,同时能明显降低血浆BNP水平,改善心功能。
Objective To observe the therapeutic efficacy of acupuncture plus Yi Qi Wen Yang decoction for chronic heart failure (CHF) due to heart-kidney yang deficiency and its effect on plasma B-type natriuretic peptide (BNP) and heart function. Method One hundred patients with CHF due to heart-kidney yang deficiency were randomized into two groups. Fifty cases in the control group were given Yi Qi Wen Yang decoction, and 50 cases in the observation group were intervened by acupuncture plus Yi Qi Wen Yang decoction, both for four consecutive weeks. The clinical efficacies of the two groups were then compared. The post-treatment changes in symptom and sign score of traditional Chinese medicine (TCM), plasma BNP, heart function and quality of life (QOL) were statistically analyzed. Result The total effective rate was 94.0% in the observation group, significantly higher than 76.0% in the control group (P<0.05);the TCM symptom and sign score, plasma BNP concentration, left ventricular end-diastolic dimension (LVEDd) and QOL score declined significantly in the two groups after the intervention (P<0.05), and the observation group was lower than the control group (P<0.05);the left ventricular ejection fraction (LVEF), stroke volume (SV) and cardiac output (CO) increased significantly after the treatment in both groups (P<0.05), and the observation group was higher than the control group (P<0.05). Conclusion Acupuncture plus Yi Qi Wen Yang decoction can produce valid efficacy for CHF due to heart-kidney yang deficiency;it can significantly down-regulate the plasma BNP concentration and improve heart function.
