摘要
目的系统评价针刺辅助治疗慢性心力衰竭(CHF)病人的临床疗效。方法计算机检索PubMed、EMbase(OVID)、Cochrane图书馆、中国知识资源总库(CNKI)、中国科技期刊数据库(VIP)、中国学术期刊数据库(万方)和中国生物医学文献数据库(CBM),检索时间自建库至2020年6月1日,由两名研究者独立筛选文献、提取资料并评价文献研究风险。采用RevMan 5.3进行Meta分析。结果共纳入19项研究,涉及1676例CHF病人。Meta分析结果显示,针刺治疗可改善CHF病人心力衰竭疗效[RR=1.19,95%CI(1.13,1.25),P<0.00001]、左室射血分数[MD=5.54,95%CI(4.21,6.87),P=0.002]、6 min步行试验距离[MD=39.48,95%CI(25.59,53.38),P<0.00001]、血清脑钠肽[MD=-171.70,95%CI(-252.97,-90.43),P<0.0001],降低明尼苏达心力衰竭生活质量调查表(MLHFQ)评分[MD=-11.36,95%CI(-17.51,-5.21),P=0.0003],提高舒张早期最大血流速度/舒张晚期最大血流速度比值[MD=0.14,95%CI(0.10,0.18),P<0.00001]。结论现有证据表明,针刺治疗可提高CHF病人临床疗效、左室射血分数及生活质量。
Objective To systematically evaluate the clinical efficacy of acupuncture therapy in patients with chronic heart failure.Methods PubMed,EMbase(OVID),Cochrane Library,CNKI,VIP,Wanfang,and CBM database were searched by computer from the establishment of the database to June 1,2020.Two researchers independently screened the literature,extracted the data and evaluated the risk of literature research.RevMan 5.3 was used for Meta-analysis.Results Nineteen studies were included involving 1676 patients with heart failure.The Meta-analysis results show that acupuncture can improve the curative effect of heart failure[RR=1.19,95%CI(1.13,1.25),P<0.00001],left ventricular ejection fraction[MD=5.54,95%CI(4.21,6.87),P=0.002],six-minute walking distance[MD=39.48,95%CI(25.59,53.38),P<0.00001],and serum brain natriuretic peptide[MD=-171.70,95%CI(-252.97,-90.43),P<0.0001],reduce Minnesota Living with Heart Failure Questionnaire(MLHFQ)score[MD=-11.36,95%CI(-17.51,-5.21),P=0.0003],increase the ratio of peak early mitral velocity[MD=0.14,95%CI(0.10,0.18),P<0.00001].Conclusion The available evidence suggests that acupuncture therapy can improve the clinical efficacy,ejection fraction,and quality of life.
