磁共振表观扩散系数对前列腺癌的筛查价值 被引量:2

摘要 目的探讨1.5 T磁共振表观扩散系数(ADC)值对前列腺癌(PCA)的筛查价值。方法选取2015年1月~2018年7月本院病理确诊的PCA患者40例作为PCA组,同期选取前列腺良性病变患者60例作为良性组,两组均给予1.5 T-MRI检查,比较两组前列腺影像报告和数据系统(PI-RADS)评分及ADC值、eADC值。结果 PCA组eADC值高于良性组、ADC值低于良性组,差异有统计学意义(P<0.05),PCA组与良性组PI-RADS评分差异有统计学意义(P<0.05);ROC曲线结果显示,在诊断PCA敏感度、特异度、准确度方面,PI-RADS评分以>3分为临界值为55.00%、83.33%、72.00%,ADC值以<0.95 mm2/s为临界值为85.00%、75.00%、79.00%,PI-RADS评分联合ADC值为95.00%、70.00%、80.00%,二者联合敏感度高于二者单独,其准确度及Youden指数也大于二者单独。结论 1.5 T-MRI的ADC值可作为PCA筛查的重要指标,且可有效提高PI-RADS评分对PCA的筛查效能,值得临床作进一步推广。 Objective To explore the value of 1.5 T magnetic resonance (MRI) apparent diffusion coefficient (ADC) in screening prostate cancer (PCA).Methods Forty patients with PCA were selected from January 2015 to July 2018 in our hospital as PCA group.At the same time,60 benign prostate lesions were selected as benign group.The patients in two groups were given 1.5 T-MRI examination.The prostate imaging reporting and data system (PI-RADS) scores,anisotropy ADC and eADC values of the two groups were compared.Results The eADC value in group PCA was significantly higher than that in benign group (P<0.05).The difference of PI-RADS score between group PCA and benign group was significant (P<0.05).In the diagnosis of PCA sensitivity,specificity and accuracy,the critical value of the PI-RADS score were 55.00%,83.33%,72.00%,and<0.95 mm^2/s as the critical value of 85.00%,75.00%,79.00%.The PI-RADS score combined with ADC value 95.00%,70%,80.00%.The sensitivity of combination of the two was significantly higher than that of the two alone.The accuracy and Youden index were also greater than the two alone.Conclusion The ADC value of 1.5 T-MRI can be used as an important indicator of PCA screening.It can effectively improve the screening effect of PI-RADS score on PCA.It is worth for further clinical promotion.
