摘要
目的基于第2版前列腺影像报告和数据系统(PI-RADS)评估多参数磁共振成像(mp-MRI)对中央腺体前列腺癌(CG-PCa)的诊断价值。资料与方法前瞻性分析62例行前列腺mp-MRI并经穿刺病理证实的前列腺中央腺体疾病患者的MRI资料,根据病变性质分为22例CG-PCa患者和40例良性前列腺增生(BPH)患者,mp-MRI参数包括T2WI、扩散加权成像(DWI)和动态对比增强MRI(DCE-MRI),以第2版PI-RADS为各独立及联合技术方案依据,分别对其扫描参数设计及影像资料进行判读评分,即T2WI、DWI、DCE-MRI和T2WI+DWI。分别对各方案的PI-RADS评分结果进行分析,计算每种方案的诊断敏感度、特异度和准确度,以ROC曲线下面积比较不同方案的诊断效能。结果依据第2版PI-RADS评分,1-3分:T2WI 39例,DWI 36例;4-5分:T2WI 23例,DWI 26例;DCE-MRI:30例为阴性,32例为阳性。T2WI、DWI、DCE-MRI和T2WI+DWI诊断CG-PCa的敏感度、特异度、准确度和ROC曲线下面积分别为72.73%、80.00%、77.42%、0.840,81.82%、72.50%、75.81%、0.827,77.27%、62.50%、67.74%、0.699,90.91%、77.50%、82.26%、0.890。结论 T2WI单独诊断CG-PCa的价值高于DWI和DCE-MRI,基于第2版PI-RADS的T2WI联合DWI对CG-PCa的诊断价值最高。
Purpose To evaluate the value of multiparametric prostate MRI(mp-MRI) in diagnosing central gland prostate cancer(CG-PCa) based on the prostate imaging-reporting and data system(PI-RADS) version 2. Materials and Methods Prospectively analyze 62 patients who underwent prostate mp-MRI consisting of T2 WI, diffusion-weighted imaging(DWI) and dynamic contrast-enhanced(DCE-MRI). Among them, 22 were confirmed CGPCa and 40 were confirmed benign prostatic hyperplasia by biopsy. T2WI, DWI, DCEMRI and T2WI+DWI were scored based on the PI-RADS version 2, as well as calculating their sensitivity, specificity and accuracy. Meanwhile, pathological results were scored based on Gleason standard. The area under ROC curves(AUC) was used to analyze the diagnostic efficiency of different protocols. Results Thirty-nine patients were scored 1-3 and 23 were scored 4-5 in T2WI; 36 were scored 1-3 and 26 were scored 4-5 in DWI; 30 were positive and 32 were negative in DCE-MRI. The sensitivity, specificity, accuracy and AUC for the diagnosis of CG-PCa were 72.73%, 80.00%, 77.42% and 0.840 for T2WI alone; 81.82%, 72.50%, 75.81% and 0.827 for DWI alone; 77.27%, 62.50%, 67.74% and 0.699 for DCE-MRI alone; 90.91%, 77.50%, 82.26% and 0.890 for T2WI+DWI. Conclusion Compares with DWI or DCE-MRI alone, T2WI improved the diagnostic performance of CG-PCa, especially combined with DWI.
