摘要
目的探讨2型糖尿病(T2DM)患者心率变异性与胰岛素抵抗(IR)、血清胱抑C(Cys-C)的相关性。方法收集2014年1月至2018年8月在郑州大学第一附属医院内分泌科住院治疗的T2DM患者95例,根据动态心电图全部窦性心搏RR间期的标准差(SDNN)结果将患者分为心率变异性正常组(n= 56)和心率变异性降低组(n=39),收集血清Cys-C、空腹胰岛素、空腹血糖等指标并计算胰岛素抵抗指数(HOMA-IR)。结果心率变异性降低组HOMA-IR及血清Cys-C水平均高于心率变异性正常组(P<0.01)。校正病程后偏相关分析显示HOMA-IR、Cys-C与心率变异性参数SDNN呈负相关(P<0.05)。结论T2DM患者心率变异性降低与胰岛素抵抗、血清胱抑素C水平密切相关,提示高Cys-C及IR与糖尿病心脏自主神经病变(DCAN)相关。
Objective To investigate the relationship between heart rate variability and insulin resistance (IR), serum cystatin C (Cys-C) in patients with type 2 diabetes mellitus (T2DM). Methods A total of 95 T2DM patients hospitalized in the Department of Endocrinology of the First Affiliated Hospital of Zhengzhou University from January 2014 to August 2018 were selected. According to the standard deviation (SDNN) of RR interval of all sinus beats in dynamic electrocardiogram, the patients were divided into normal heart rate variability group (n=56) and reduced heart rate variability group (n=39). The levels of serum Cys-C, fasting insulin and fasting blood glucose were observed and the level of insulin resistance index (HOMA-IR) was calculated. Results The HOMA-IR and serum Cys-C levels in patients with reduced heart rate variability were higher than those in patients with normal heart rate variability (P<0.01). After correcting the duration of diabetes mellitus, partial correlation analysis showed that HOMA-IR and Cys-C were negatively correlated with SDNN (P<0.05). Conclusions Reduced heart rate variability in type 2 diabetes mellitus is closely correlated with insulin resistance and serum Cys-C levels, suggesting that the high levels of Cys-C and insulin resistance are correlated with diabetic cardiac autonomic neuropathy.
