血清骨桥蛋白、神经胶质纤维酸性蛋白及生长分化因子15水平在急性脑梗死患者神经损伤及预后评估中的应用价值 被引量:18

摘要 目的探讨血清骨桥蛋白(OPN)、神经胶质纤维酸性蛋白(GFAP)及生长分化因子15(GDF-15)水平在评估急性脑梗死患者神经损伤及预后中的应用价值。方法选择郑州人民医院2015年5月至2017年7月收治的65例急性脑梗死患者为研究对象(脑梗死组),并选择同期体检健康者50例为对照组。根据美国国立卫生研究院卒中量表(NIHSS)评分和改良Rankin量表(mRS)评分将脑梗死组患者再分为NIHSS<7分组和NIHSS≥7分组、mRS≤2分组和mRS>2分组。采用酶联免疫吸附试验检测各组受试者血清OPN、GFAP及GDF-15水平,并比较各组受试者血清OPN、GFAP及GDF-15水平的差异及OPN、GFAP、GDF-15水平与NIHSS、mRS评分的相关性。结果脑梗死组患者血清OPN、GFAP及GDF-15水平均显著高于对照组(P<0.01)。NIHSS≥7分组患者血清OPN、GFAP及GDF-15水平显著高于NIHSS<7分组(P<0.01)。mRS>2分组患者血清OPN、GFAP及GDF-15水平显著高于mRS≤2分组(P<0.05)。急性脑梗死患者血清OPN、GFAP及GDF-15水平与NIHSS、mRS评分均呈显著正相关(P<0.05)。结论联合检测血清OPN、GFAP及GDF-15水平可作为评估急性脑梗死患者神经功能损伤及预后的参考指标。 Objective To investigate the value of serum osteopontin (OPN),neurofibrillar acidic protein (GFAP) and growth differentiation factor15 (GDF-15) in the assessment of nerve injury and prognosis in patients with acute cerebral infarction. Methods Sixty-five patients with acute cerebral infarction admitted to Zhengzhou People′s Hospital from May2015 to July2017 were selected as the study subjects (cerebral infarction group),and fifty healthy subjects during the same period were selected as the control group.According to the score of national institutes of health stroke scale (NIHSS) and modified Rankin scale (mRS),the patients in the cerebral infarction group were divided into NIHSS <7 group and NIHSS 7≥ group,mRS≤2 group and mRS >2 group.The serum OPN,GFAP and GDF-15 levels were detected by enzyme-linked immuno sorbent assay.The serum levels of OPN,GFAP and GDF-15 were compared among the groups,and the correlations between the levels of OPN, GFAP,GDF-15 and the NIHSS,mRS scores were analyzed. Results The serum levels of OPN,GFAP and GDF-15 in the cerebral infarction group were significantly higher than those in the control group ( P <0.01).The serum levels of OPN,GFAP and GDF-15 in the NIHSS≥7 group were significantly higher than those in the NIHSS<7 group ( P <0.01).The serum levels of OPN,GFAP and GDF-15 in mRS>2 group were significantly higher than those in mRS≤2 group ( P <0.05).The serum levels of OPN,GFAP and GDF-15 were positively correlated with NIHSS and mRS scores in patients with acute cerebral infarction ( P <0.05). Conclusion Combined detection of serum OPN,GFAP and GDF-15 levels can be used as a reference index to evaluate the neurological damage and prognosis of patients with acute cerebral infarction.
