Analysis and evaluation of the operation of Liaoning polio laboratory (2014-2017)

摘要 目的评价2014—2017年辽宁省脊灰实验室网络运转情况,为制定免疫策略及维持辽宁无脊灰状态提供依据。方法用病毒分离方法对急性弛缓性麻痹(acute flaccid paralysis,AFP)病例的粪便标本进行检测,用rRT-PCR方法对脊灰病毒进行型内鉴定,并对VP1基因编码区进行序列测定;对WHO要求的各项监测指标进行评价;对实验室的质控和考核进行分析评价。结果 421份病例标本中有8例分离到脊灰病毒,分离率为1.90%,其中,Ⅱ型、Ⅲ型各1例,Ⅰ型和混合型各3例;35例标本中分离到非脊灰肠道病毒(non-polio enterovirus,NEPV),分离率为8.31%。脊灰病毒VP1编码区测序结果显示,7例为疫苗类似株,1例为疫苗衍生脊髓灰质炎病毒(vaccine-derived poliovirus, VDPV)。各项监测指标均达到WHO的要求;细胞对脊灰病毒敏感、WHO的各项考核取得优异成绩。结论 2014—2017年辽宁省脊灰实验室运转正常,为维持辽宁省无脊灰状态提供了可靠的实验室依据。 Objectives To evaluate the operation of Liaoning polio laboratory from 2014 to 2017, and to provide basis for making immunization strategies and maintaining polio-free status in Liaoning Province. Methods Stool specimens from acute flaccid paralysi (AFP) cases were detected by virus isolation, and poliovirus was identified by rRT-PCR, and the VP1 gene coding region was sequenced. The surveillance indicators required by WHO were analyzed. The laboratory quality control and assessment were evaluated. Results 8 of 421 cases were poliovirus positive, and the positive isolation rate was 1.90%. Among them, there were 1 PV II and 1 PV III, 3 PV I and 3 PV mixed. Non-polio enterovirus (NPEV) was isolated from 35 specimens, and the isolation rate was 8.31%. Poliovirus sequencing results of VP1 coding region showed that 7 cases were sabin-like strains and 1 case was vaccine-derived poliovirus (VDPV). All monitoring indicators met WHO requirements. Cells were sensitive to polioviruses, and excellent results have been achieved in various WHO assessments. Conclusion Liaoning polio laboratory operated well from 2014 to 2017. It provides a reliable basis for maintaining polio-free status in Liaoning Province.
