摘要
目的探讨慢性阻塞性肺病(COPD)患者呼出气冷凝液IL-33的表达水平与临床特征及气道嗜酸性粒细胞的关系。方法选择2017年9月至2018年7月于我院就诊的COPD患者共60例,同时选择健康参与者50例作为对照组,收集所有患者呼出气冷凝液、诱导痰及血清,采用ELISA法检测呼出气冷凝液的IL-33的表达水平,检测血清IL-33、IL-6、IL-8、TNF-α的表达水平,对诱导痰嗜酸性粒细胞进行计数,分析IL-33与临床特征、嗜酸性粒细胞计数及炎症指标的关系。结果 COPD组的平均呼出气冷凝液IL-33水平显著高于对照组(P <0.05),AECOPD患者呼出气冷凝液IL-33水平显著高于稳定期的患者(P <0.001),且不同GOLD分级COPD患者的呼出气冷凝液IL-33水平对比差异具有统计学意义(P=0.004),呼出气冷凝液IL-33水平与血清IL-33、气道嗜酸性粒细胞比例、血嗜酸性粒细胞比例、IL-6、IL-8和TNF-α呈显著的正相关,呼出气冷凝液IL-33水平与肺功能指标中的FEV1/FVC呈显著的负相关(r=-0.392,P=0.002)。结论 COPD患者呼出气冷凝液IL-33的表达水平显著增高,气道IL-33参与了COPD患者的病情进展,且与气道中的嗜酸性粒细胞比例及血清IL-6、IL-8和TNF-α水平显著相关。
Objective To investigate the relationship between the expression level of exhaled air conden sate IL-33 and clinical features and airway eosinophils in patients with COPD. Methods Sixty COPD patients ad mitted to our hospital from September 2017 to July 2018 were enrolled as COPD group and 50 healthy participants were selected as the control group. Exhaled condensate,induced sputum and serum were collected in all the pa tients. The expression level of IL-33 in exhaled breath condensate was detected,and the expression levels of serum IL-33,IL-6,IL-8 and TNF-α were detected. The induced eosinophils were counted,and the relationship among IL-33 and clinical characteristics,eosinophil count and inflammation indicators was analyzed. Results The average exhaled breath condensate IL-33 level in COPD group was significantly higher than that in the control group(P<0.05) and the level of exhaled effluent IL-33 in patients with AECOPD was significantly higher than that in stable patients(P<0.001). There was significant difference in the level of exhaled breath condensate IL-33 between pa tients with different grades of COPD(P = 0.004). The level of exhaled condensate IL-33 was significantly positive ly correlated with serum IL-33,airway eosinophil ratio,blood eosinophil ratio,IL-6,IL-8 and TNF-α. There was significantly negative correlation between IL-33 level and FEV1/FVC(r =-0.392,P = 0.002). Conclusion The expression level of exhaled air condensate IL-33 is significantly increased in patients with COPD. Airway IL-33 is involved in the progression of COPD patients,and the ratio of eosinophils in the airway and serum IL-6,IL-8 and TNF-α levels are significantly correlated.
