摘要
目的 探讨慢性阻塞性肺疾病(COPD)患者血清降钙素原(PCT)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)水平与肺功能之间的关系.方法 选择2012年12月至2016年12月长治市第二人民医院186例COPD合并肺部感染患者,根据GOLD分级,按患者肺功能将其分为四组:Ⅰ级组、Ⅱ级组、Ⅲ级组、Ⅳ级组,检测患者血清PCT、IL-6、TNF-α和CRP水平反映患者肺部感染情况,比较患者感染前后临床症状和生活质量评分,比较各组患者血清PCT、IL-6、TNF-α和CRP水平之间的差异,分析这些指标与肺功能之间的相关性.结果 186例COPD合并肺部感染患者感染前临床症状评分[(3.65±0.81)分]和生活质量评分[(6.32±0.97)分]均低于感染后两者评分[(5.69±1.37)分,(7.86±1.37)分],差异有统计学意义,P<0.05.四组患者血清PCT、IL-6、TNF-α和CRP水平均随组别升高而升高,且这些指标与肺功能之间均呈负相关(P<0.05).结论 COPD合并肺部感染导致患者临床症状恶化和生活质量下降,且感染严重程度与肺功能呈负相关.
Objective To investigate the relationship between serum levels of PCT,IL-6,TNF-α and CRP and pulmonary function in patients with COPD infection.Methods A total of 186 COPD patients with pulmonary infection in the Second People's Hospital of Changzhi from December 2012 to December 2016 were divided into four groups according to GOLD classification:grade Ⅰ group,grade Ⅱ group,grade Ⅲ group and grade Ⅳ group,serum levels of PCT,IL-6,TNF-and CRP were detected in patients with pulmonary infection,the clinical symptoms and quality of life before and after infection were compared,the serum levels of PCT,IL-6,TNF-alpha and CRP in each group were compared,and the correlation between these indexes and pulmonary function was analyzed.Results The clinical symptoms scores before infection (3.65 ± 1.37)and quality of life score (6.32 ± 0.9) in 186 COPD patients with pulmonary infection were lower than those after infection [(5.69 ± 1.37),(7.86 ± 1.37)].The difference was statistically significant (P < 0.05).Serum PCT,IL-6,TNF-alpha and CRP levels in the four groups increased with the increase of the groups,and there were negative correlations between these indexes and pulmonary function (P < 0.05).Conclusions COPD complicated with pulmonary infection leads to worsening of clinical symptoms and lower quality of life,and the severity of infection is negatively correlated with lung function.
