摘要
目的分析康复训练联合高压氧治疗对急性缺血性脑卒中患者脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)及肌力的影响。方法选取本院2016年5月至2018年2月收治的急性缺血性脑卒中患者116例为研究对象,采用随机数表法将其分为观察组和对照组,每组各58例。对照组患者采取常规治疗联合康复训练,观察组患者在对照组治疗基础上联合高压氧治疗。测定并比较两组患者治疗前后血清BDNF水平及偏瘫侧肢体肌力的差异。结果治疗后两组患者血清BDNF水平均显著高于本组入院时(P_均<0.05)。治疗后第7天和第15天,观察组患者血清BDNF水平均显著高于同期对照组(P_均<0.05)。康复训练8周后,两组患者的髋关节前屈(force hip flex,FHF)、髋关节后伸(force hip ext,FHE)肌力和膝关节前屈(force knee flex,FKF)、髋关节后伸(force knee ext,FKE)肌力与康复训练前比较均显著增加(P_均<0.05),且观察组患者FHE、FKE肌力均显著大于对照组(P_均<0.05)。结论康复训练联合高压氧治疗可以提高急性缺血性脑卒中患者血清BDNF水平,有助于促进缺氧脑组织修复,增加偏瘫侧肢体肌力。
Objective To explore the effect of rehabilitation training combined with hyperbaric oxygenation on brain-derived neurotrophic factor(BDNF)and muscle strength in patients with acute ischemic stroke(AIS).Method 116 patients with AIS treated in our hospital from May 2016 to February 2018 were randomly divided into observation group and control group by random number method,58 cases in each group.Patients in control group were treated by conventional therapy combined with rehabilitation training,observation group patients were treated by hyperbaric oxygenation on the basis of control group.The levels of BDNF and limb muscle strength were measured and compared before and after treatment between the two groups.Result After treatment,the level of serum BDNF in both groups were significantly higher than that at admission(Pall<0.05).On the 7th and 15th day after treatment,the levels of serum BDNF in observation group were significantly higher than that in control group(Pall<0.05).After 8 weeks of rehabilitation training,the force hip flex(FHF),force hip ext(FHE),force knee flex(FKF),force knee ext(FKE)muscle strength of the two groups were significantly increased compared with those before rehabilitation training(Pall<0.05),and the FHE and FKE muscle strength of observation group were significantly higher than those of control group(Pall<0.05).Conclusion Rehabilitation training combined with hyperbaric oxygenation can increase the levels of serum BDNF in AIS patients,help to repair hypoxic brain tissue and increase the muscle strength of hemiparalysis.
