摘要
目的建立静脉治疗门诊延伸护理服务模式,使地区内携带静脉导管的患者能就近维护,以降低治疗成本,降低导管维护期间并发症发生率。方法通过在地区内建立导管维护三级网络,评价置管患者返院维护率及留置期间并发症[导管相关感染、医用黏胶相关性皮肤损伤(MARSI)、导管堵塞]发生率。结果2015年上半年患者返院维护率60.0%,2016年下半年返院维护率29.7%,差异有统计学意义(P=0.000),2015年上半年导管留置期间导管相关感染的发生率由8.6%下降至3.3%,差异有统计学意义(P=0.000),MARSI发生率由29.7%下降为10.1%,差异有统计学意义(P=0.000),导管堵塞发生率由8.5%下降为0.6%,差异有统计学意义(P=0.000)。结论静脉治疗门诊综合延伸服务模式降低导管维护期间的并发症发生率,在降低静脉治疗门诊工作量的同时,方便患者就近维护,值得临床推广。
Objective To establish an extended nursing service model for intravenous treatment in outpatient, so that patients with venous catheters nearby can receive catheter maintenance, which can reduce the medical cost and the incidence of complications during the maintenance of the catheters. Methods A three-level network of catheter maintenance was established in the area to evaluate the rate of return maintenance and the incidence of complications [catheter-related infection, medical adhesive-related skin injuries (MARSI) and catheter blockage] during catheterization. Results The rate of patient return maintenance as statistically significant between the first half of 2015 and the second half of 2016 (60.0% vs. 29.7%, P=0.000). Compared with the first half of 2015, the incidence of catheter-related infections decreased in the second half of 2016 from 8.6% to 3.3%, the incidence of MARSI decreased from 29.7% to 10.1%, and the incidence of catheter blockage decreased from 8.5% to 0.6%.(All P=0.000). Conclusion The comprehensive extended service mode for outpatient with venous catheters can reduce the incidence of complications during catheter maintenance, which can reduce the workload of outpatient intravenous treatment and also provide patients the convenience of catheter maintenance, and is worthy of clinical promotion.
