摘要
目的探讨快速进展期白癜风口服复方中药联合激素和免疫调节剂的临床疗效与抗酪氨酸酶抗体(TYR IgG)和抗酪氨酸酶相关蛋白-1抗体(TRP-1 IgG)检测结果的关系。方法 68例门诊快速进展期白癜风患者治疗前后用酶联免疫吸附法检测血清TYR IgG和TRP-1 IgG的含量。治疗用复方倍他米松注射液1 m L深部肌肉注射,外用0.1%他克莫司软膏,口服复方中药祛白颗粒。结果白癜风快速进展期治疗有效率为89.71%(61/68)。治疗后TYR IgG和TRP-1 IgG含量较治疗前显著降低(P<0.05)。儿童组(≤14岁)治疗后TYR IgG和治疗前及治疗后TRP-1 IgG水平均低于>14岁组(P<0.05)。面颈部白癜风治疗效果最好。治疗方法不良反应很轻,不影响治疗过程。结论 TYR IgG和TRP-1 IgG检测结果与白癜风治疗效果密切相关,为白癜风的疗效提供了证据,具有指导继续治疗的应用价值。应用激素、免疫调节剂和复方中药治疗进展期白癜风有较好的疗效。
Objective In order to investigate the correlation between the levels of anti-tyrosinase IgG antibody(TYR IgG),anti-tyrosinase-related protein-1 IgG antibody(TRP-1 IgG)and the therapeutic efficacy of traditional Chinese medicine(TCM)combined hormone,immunomodulator for rapid progressive stage vitiligo.Methods In 68 outpatients with rapid progressive stage vitiligo,the serum levels of TYR IgG and TRP-1 IgG were detected by ELISA before and after treatment,and they were treated by compound betamethasone 1 mL intramuscular injection,smearing topical 0.1%tacrolimus ointment,and oral compound TCM.Results The effective rate of treatment for 68 cases was 89.71%.The levels of TYR IgG and TRP-1 IgG both decreased significantly after treatment(all P<0.05).The levels of TYR IgG after treatment as well as those of TRP-1 IgG before and after treatment of≤14 years old group was lower than that of >14 years old group(all P<0.05).The facial and cervical vitiligo had the best therapeutical efficacy.The side effects of treatment were minimal and didn′t affect the course of treatment.Conclusion The variation of TYR IgG and TRP-1 IgG had close correlation to the therapeutic effects of vitiligo.The detection of TYR IgG and TRP-1IgG may provide the evidence to estimate therapeutic effect of vitiligo,and have the clinic value of directing continuous treatment.The combination of compound TCM,hormone andimmunomodulator are more effective in treating vitiligo.
