摘要
目的分析2017年12月贵州省肿瘤医院医务人员维生素D的营养状态。方法研究977名医务人员的维生素D营养状况及影响因素,分析血清25-羟维生素D(25OHD)与葡萄糖(GLU)、肌酐(Crea)、尿酸(UA)、甘油三酯(TG)、胆固醇(TC)和丙氨酸氨基转移酶(ALT)的相关性。结果 977名医务人员血清250HD水平为48. 2(5. 0~175. 0)nmol/L,维生素D缺乏(250HD <50nmol/L)比率为53. 8%。女性血清250HD水平低于男性(P <0. 05),维生素D缺乏率高于男性(P <0. 05)。男性血清250HD水平在不同年龄组间差异无统计学意义(P <0. 05)。女性血清250HD水平随年龄的增长而升高,≥50岁组高于<30岁组和30~39岁组(P<0.05)。护士组血清250HD水平低于医生组和行政后勤组(P<0.05)。在控制性别、年龄和岗位后,血清250HD与Crea、UA和TC呈正相关(r分别为0. 17、0. 08和0. 07,P <0. 05),与TG呈负相关(r=-0.08,P<0.05)。结论贵州省肿瘤医院医务人员存在维生素D缺乏。性别、年龄和岗位是影响医务人员维生素D营养状态的因素,特别是青年女性和护士。在医务人员的健康管理中,应定期评价维生素D的营养状态,同时监测肾功能和脂类代谢。
Objective To analyze the vitamin D status among the medical personnel of Guizhou Cancer Hospital in December 2017. Methods The study participant included 977 medical personnel. The goal was to estimate the vitamin D status and its influencing factors in subjects and analyze the correlation between serum25-hydroxy vitamin D(250 HD)and GLU,Crea,UA,TG and TC. Results The median serum 250 HD levels of 977 medical personnel was 48.2 nmol/L(range 5.0-175. 0). The rate of vitamin D deficiency(25 OHD<50 nmol/L) was 53.8%. Female subjects had a significantly lower serum level of 250 HD(P <0. 05),and a significantly higher rate of vitamin D deficiency(P < 0.05). There was no significant difference in serum level of 250 HD among different age groups in male subjects(P <0.05). As age increased,the serum level of25 OHD increased gradually in female subjects and the serum level of 25 OHD in-50 years group was significantly higher than <30 years group and 30-39 years group(P <0. 05). The serum level of 250 HD in nurse group was significantly lower than the physician group and the administrative group(P <0. 05). After controlling sex,age and career position,serum 25 OHD had a significant positive correlation with Crea,UA,and TC( r was 0.17,0.08,0.07,respectively,P <0. 05),and a significant negative correlation with TG(r =-0.08,P <0.05). Conclusion Vitamin D deficiencies are prevalent among the medical personnel of Guizhou Cancer Hospital. The subjects ’ sex, age and career position were the factors affecting vitamin D status of medical personnel, especially among young females and nurses. For the future health control of medical personnel, the vitamin D status should be evaluated regularly, and the renal function and lipid metabolism should be monitored.
