摘要
目的:分析支气管哮喘患者脂联素水平变化情况及其临床意义。方法:选取本院2015年7月-2018年6月收治的220例支气管哮喘患者作为观察组,同时选取同期220例健康体检者作为对照组。比较两组的脂联素浓度、肿瘤坏死因子α(TNF-α)、肺功能、动脉血气指标水平,并分析脂联素浓度与上述指标之间的关系。结果:观察组脂联素浓度为(3.23±1.27)ng/m L明显低于对照组的(9.47±3.08)ng/m L,TNF-α水平为(125.29±52.25)pg/mL明显高于对照组的(65.63±15.98)pg/mL,差异均有统计学意义(P<0.05)。两组PaCO_2、PaO_2水平比较,差异均无统计学意义(P>0.05);但观察组FEV_1%值明显低于对照组,RV%值明显高于对照组,差异均有统计学意义(P<0.05)。观察组脂联素浓度与FEV_1%值呈正相关(r=0.448,P<0.05),与RV%值、TNF-α水平呈负相关(r=-0.368、-0.392,P<0.05)。结论:支气管哮喘患者血清脂联素水平明显降低、TNF-α水平升高、FEV_1%值降低,脂联素参与支气管哮喘患者的发病过程,在其中可能发挥一定保护作用。
Objective:To analyze the changes and clinical significance of adiponectin levels in patients with bronchial asthma.Method:A total of 220 patients with bronchial asthma admitted in in our hospital from July 2015 to June 2018 were selected as the observation group,and 220 healthy people in the same period were selected as the control group.The levels of adiponectin,TNF-alpha,pulmonary function and arterial blood gas between the two groups were compared,and the relationship between adiponectin concentration and the above indexes was analyzed.Result:The concentration of adiponectin in the observation group was(3.23±1.27) ng/mL,which was significantly lower than(9.47±3.08) ng/mL in the control group,the level of TNF-α was(125.29±52.25) pg/mL,which was significantly higher than(65.63±15.98) pg/mL in the control group,the differences were statistically significant(P<0.05).There were no significant differences in PaCO2 and PaO2 levels between the two groups(P>0.05);but the value of FEV1% in the observation group was significantly lower than that in the control group,the value of RV% was significantly higher than that in the control group,the differences were statistically significant(P<0.05).The concentration of adiponectin in the observation group was positively correlated with FEV1% value(r=0.448,P<0.05),negatively correlated with RV% value and TNF-α level(r=-0.368,-0.392,P<0.05).Conclusion:The level of serum adiponectin in patients with bronchial asthma decreased significantly,the level of TNF-α increased,and the value of FEV1% decreased.Adiponectin may play a protective role in the pathogenesis of bronchial asthma.
