摘要
目的观察血液灌流联合血液透析治疗急诊重症有机磷农药中毒(AOPP)的临床效果。方法选择2017年1～12月我院收治的AOPP患者118例为研究对象,按随机数字表法分为对照组和观察组各59例,对照组接受洗胃、吸氧、静脉推注阿托品等常规治疗,观察组加用血液灌流、血液透析治疗,观察比较两组患者的临床相关指标、谷丙转氨酶(ALT)、肌酸激酶(CK)、乳酸脱氢酶(LDH)水平及并发症发生情况。结果观察组患者的胆碱酯酶(CHE)恢复时间(5.79±4.01)天、住院时间(10.29±2.51)天、阿托品用量(163.98±15.06)mg均少于对照组,ALT(46.06±3.73)U/L、CK(98.53±8.03)U/L、LDH(204.72±32.51)U/L均低于对照组,差异均有统计学意义(P<0.05)。与对照组比较,观察组的并发症发生率3.39%、病死率3.39%相对较低,差异均有统计学意义(P<0.05)。结论 AOPP患者使用血液灌流、血液透析联合治疗可减少相关并发症,减轻机体重要脏器的损伤,提高抢救成功率,促进患者恢复。
Objective To observe the clinical effect of blood perfusion combined with hemodialysis in treatment of severe acute organic phosphorus poisoning(AOPP).Methods118AOPP patients treated in the hospital from January to December2017were divided into two groups by random number table,with59cases in each group.Control group was given routine treatment with gastric lavage,oxygen uptake,intravenous injection of atropine,while observation group was treated with blood perfusion and hemodialysis based on control group.The clinical indexes,alanine transaminase(ALT),creatine kinase(CK),lactate dehydrogenase(LDH),complications in two groups were observed.Results Compared with control group,the recovery time of cholinesterase(CHE),hospital stays,dose of atropine in observation group[(5.79±4.01)d,(10.29±2.51)d,(163.98±15.06)mg]were less,the levels of ALT,CK,LDH in observation group[(46.06±3.73)U/L,(98.53±8.03)U/L,(204.72±32.51)U/L]were lower(P<0.05);the incidence of complications and case fatality rate in observation group(3.39%,3.39%)were lower than those in control group(P<0.05).Conclusion Blood perfusion combined with hemodialysis in treatment of AOPP can reduce complications,shorten hospital stays,promote recovery of CHE,decrease organ injury and increase the rescue success rate.
