摘要
目的通过分析肺癌患者血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平与凝血指标活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白原(FIB)、D-二聚体(DD)、血小板(PLT)水平的变化,探讨癌症患者炎症反应与凝血指标变化的关系。方法选择65例肺癌患者作为观察组,选取同期50例健康体检人员作为对照组,比较2组受试人员的血清TNF-α、IL-6水平和凝血指标,分析癌症患者炎症反应与凝血功能的相关性。结果对照组血清TNF-α、IL-6水平显著低于各个时期的观察组(P均<0. 05)。对照组FIB、DD浓度显著低于各个时期的观察组(P均<0. 05);对照组APTT、PLT水平显著高于各个时期的观察组(P均<0. 05);对照组PT水平显著低于观察组Ⅲ、Ⅳ期(P均<0. 05),但与观察组Ⅰ、Ⅱ期无统计学差异(P均> 0. 05)。血清TNF-α水平与APTT、PLT呈负相关(γ=-0. 963,P=0. 009;γ=-0. 963,P=0. 009),TNF-α与PT、FIB、DD呈正相关(γ=0. 940,P=0. 017;γ=0. 996,P=0. 000;γ=0. 987,P=0. 002)。血清IL-6水平与APTT、PLT呈负相关(γ=-0. 986,P=0. 002;γ=-0. 993,P=0. 001),IL-6与FIB、DD呈正相关(γ=0. 979,P=0. 004;γ=0. 993,P=0. 001),IL-6与PT无相关性(P> 0. 05)。结论肺癌患者体内存在炎症反应和凝血功能紊乱,且随着临床分期的增加反应加剧,并且其血清炎症因子和凝血指标之间具有相关性。
Objective To explore the relationship between inflammatory response and coagulation index in cancer patients by analyzing the changes of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and thromboplastin(APTT),prothrombin time(PT),fibrin(FIB),D-dimer(DD),platelet(PLT)levels.Methods Selected 65 patients with lung cancer as the observation group.Selected 50 healthy persons who receiving health examination as the control group at the same period.The serum TNF-α,IL-6 levels and coagulation parameters were compared between the 2 groups.To analyze the correlation between inflammatory response and coagulation function in cancer patients.Results The levels of serum TNF-αand IL-6 in the control group were significantly lower than those of the control group(P<0.05).The concentration of FIB and DD in control group was significantly lower than that of observation group(P<0.05).The levels of APTT and PLT in the control group were significantly higher than those of the observation group(P<0.05).The level of PT in the control group was significantly lower than that of the observation groupⅢandⅣ(P<0.05),but there was no significant difference with the observation groupⅠandⅡ(P>0.05).The levels of serum TNF-αwere negatively correlated with APTT and PLT(γ=-0.963,P=0.009;γ=-0.963,P=0.009).TNF-αwas positively correlated with PT,FIB,DD(γ=0.940,P=0.017;γ=0.996,P=0.000;γ=0.987,P=0.002).Serum IL-6 levels were negatively correlated with APTT and PLT(γ=-0.986,P=0.002;γ=-0.993,P=0.001).IL-6 was positively correlated with FIB and DD(γ=0.979,P=0.004;γ=0.993,P=0.001),IL-6 was not associated with PT(P>0.05).Conclusion Lung cancer patients with inflammatory response and coagulation disorders,and with the increase in clinical staging increased,and its serum inflammatory factors and coagulation indicators are related.
