摘要
目的观察针刺督脉背段联合疏肝利胆和胃中药治疗非糜烂性胃食管反流病(NERD)的临床疗效。方法将144例NERD患者随机分为观察组和对照组,每组72例。对照组给予疏肝利胆和胃中药治疗,观察组在对照组治疗的基础上给予针刺督脉背段治疗。观察两组治疗前后食管黏膜分级、胃泌素、胃动素及SCL-90各因子评分变化,并比较两组临床疗效。结果观察组与对照组总有效率分别为98.6%和86.1%,观察组优于对照组(P<0.05)。观察组治疗后的食管黏膜分级优于对照组(P<0.05)。两组治疗后血清胃泌素与胃动素水平高于同组治疗前(P<0.05),观察组高于对照组(P<0.05)。两组治疗后SCL-90各因子评分与同组治疗前比较,差异有统计学意义(P<0.05),观察组评分低于对照组(P<0.05)。治疗后随访1年,观察组与对照组的复发率分别为1.4%和11.3%,观察组低于对照组(P<0.05)。结论针刺督脉背段联合疏肝利胆和胃中药治疗NERD能提高疗效,改善胃肠动力,减少对食管黏膜的损伤,改善患者的心理状态。
Objective To observe the clinical efficacy of acupuncture at the back segment of Governor Vessel plus Chinese medication for regulating liver-gallbladder and harmonizing stomach in treating non-erosive gastroesophageal reflux disease(NERD).Method A total of 144 NERD patients were randomized into an observation group and a control group,with 72 cases in each group.The control group was given Chinese medication for regulating liver-gallbladder and harmonizing stomach,while the observation group was additionally given acupuncture at the back segment of Governor Vessel.Esophageal mucosa grading,gastrin,motilin and the component scores of Symptom Checklist-90(SCL-90)were observed before and after the treatment,and the clinical efficacies were compared between the two groups.Result The total effective rate was 98.6%in the observation group versus 86.1%in the control group,and the observation group was superior to the control group(P<0.05).The esophageal mucosa grading in the observation group was better than that in the control group after the treatment(P<0.05).The levels of gastrin and motilin in serum increased after the treatment in both groups(P<0.05),and the levels in the observation group were higher than those in the control group(P<0.05).The component scores of SCL-90 changed significantly after the treatment in the two groups(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05).The one-year follow-up showed that the relapse rate was 1.4%in the observation group versus 11.3%in the control group,and the rate in the observation group was lower than that in the control group(P<0.05).Conclusion Acupuncture at the back segment of Governor Vessel plus Chinese medication for regulating liver-gallbladder and harmonizing stomach can enhance the therapeutic efficacy in treating NERD.It can improve the gastrointestinal motility,reduce the damage to esophageal mucosa and improve the patient’s psychological state.
