摘要
目的:探讨血清成骨因子和成脂因子在2型糖尿病并发骨质疏松患者中的变化规律。方法:选择2016年4月—2017年10月盐城市第一人民医院进行双能X线骨密度(dual energy X-ray absorptiometry, DXA)检查的120例2型糖尿病患者为观察组(A组:骨密度正常的患者30例,B组:合并骨量减少的患者28例,C组:合并骨质疏松的患者33例,D组:合并骨质疏松并行强化血糖控制的患者29例),30例非糖尿病且骨密度正常的健康者为对照组,应用酶联免疫吸附法测定研究对象的碱性磷酸酶(alkaline phosphatase, ALP)、Runt相关转录因子2(Runt-related transcription factor 2,Runx2)、过氧化物酶增殖物激活受体γ(peroxisome proliferator-activated receptorγ, PPARγ)、CCAAT-增强子结合蛋白α(CCAAT/enhancer binding proteinα, C/EBPα)水平。结果:观察组各亚组糖化血红蛋白均高于对照组(P<0.05),DXA骨密度测定T值均低于对照组(P<0.05);观察组中D组糖化血红蛋白低于C组(P<0.05),DXA骨密度测定T值高于C组(P<0.05);观察组各亚组的ALP、Runx2水平均明显低于对照组(P<0.05),PPARγ、C/EBPα水平均高于对照组(P<0.05);观察组中D组患者ALP、Runx2水平高于C组(P<0.05),PPARγ、C/EBPα水平低于C组(P<0.05)。ALP、Runx2与骨密度水平呈正相关,PPARγ、C/EBPα水平与骨密度水平呈负相关。结论:血清成骨因子和成脂因子在2型糖尿病并发骨质疏松形成及发展过程中具有重要意义,骨密度水平与成骨因子呈正相关,与成脂因子呈负相关,可为骨质疏松诊断及治疗效果评估提供参考,以提高诊断准确性。
Objective:To investigate the change rules of serum osteogenic factor and adipogenic factor in type 2 diabetes mellitus with osteoporosis.Methods:From April 2016 to October 2017,a total of 120 patients with type 2 diabetes mellitus were measured bone mineral density by using dual energy X-ray absorptiometry(DXA)in the Yancheng City No.1 People′s Hospital.They were selected as the observation group(Group A:30 patients with normal bone mineral density,Group B:28 patients with bone mass loss,Group C:33 patients with osteoporosis,Group D:29 patients with osteoporosis and intensive blood glucose control).Thirty healthy patients with normal bone mineral density were selected as the control group.The levels of alkaline phosphatase(ALP),Runt-related transcription factor 2(Runx2),peroxisome proliferator-activated receptorγ(PPARγ)and CCAAT/enhancer binding proteinα(C/EBPα)were detected by enzyme-linked immunosorbent assay(ELISA).Results:The level of HbA1c in all observation groups was significantly higher than that in the control group(P<0.05),and the T-score measured by DXA in all observation groups was significantly lower compared to the control group(P<0.05).In the observation groups,the level of HbA1c in Group D was significantly lower than that in Group C(P<0.05),and the T-score measured by DXA in Group D was significantly higher than that in Group C(P<0.05).The levels of ALP and Runx2 in all observation groups were significantly lower than those in the control group(P<0.05),and the levels of PPARγand C/EBPαin the observation group were significantly higher than those in the control group(P<0.05).In the observation groups,the levels of ALP and Runx2 in Group D were significantly higher than those in Group C(P<0.05),and the levels of PPARγand C/EBPαin Group D were significantly lower than those in Group C(P<0.05).The levels of ALP and Runx2 are positively correlated with the bone mineral density.And the levels of PPARγand C/EBPαare negatively correlated with the bone mineral density.Conclusions:The levels of
