颈动脉斑形态及狭窄程度对脑梗死患者认知功能障碍的影响 被引量:2

摘要 目的:探究颈动脉斑形态及狭窄程度对脑梗死患者认知功能障碍的影响。方法:选取2016年1月至2017年1月医院收治确诊脑梗死并经彩色多普勒超声仪颈动脉超声检测出合并颈动脉粥样硬化患者100例,采用中文版简易智能精神状态检查量表(MMSE)测定患者认知功能,并按照MMSE结果分为认知功能正常组(44例)与认知障碍组(56例),分析所有患者临床资料,以及颈动脉斑形态与颈动脉腔狭窄程度与患者认知功能障碍相关性。结果:脑梗死患者治疗后发生认知障碍与患者性别、年龄、甘油三酯的水平以及颈动脉斑块形态无关(P>0.05),与高血压病程、血糖水平、胆固醇水平、颈动脉狭窄程度以及双颈动脉斑块总面积有关(P<0.05),剔除高血压病程、血糖水平、胆固醇水平等因素,颈动脉狭窄程度以及双颈动脉斑块总面积仍是脑梗死患者的危险因素(r=-0.369,r=-0.741,P<0.05)。结论:颈动脉狭窄程度以及双颈动脉斑块总面积是脑梗死患者的危险因素,临床医师应根据患者颈动脉斑块大小与形态给与相应预防措施,以提高患者治疗后生活质量。 Objective:To investigate the effect of carotid plaque morphology and degree of stenosis on cognitive dysfunction in patients with cerebral infarction.Methods:January 2016-January 2017admitted to our hospital diagnosed cerebral infarction and color Doppler ultrasound carotid artery ultrasound detected carotid atherosclerosis in patients with 100cases were selected as the research object,The cognitive function of patients was measured by MMSE and divided into normal cognitive function group(n=44)and cognitive impairment group(n=56)according to MMSE results.All patients were analyzed clinical data,as well as the relationship between carotid plaque morphology and carotid artery stenosis and patient’s cognitive dysfunction.Results:Cognitive impairment in patients with cerebral infarction was not related to gender,age,triglyceride level and carotid plaque morphology(0.05).And hypertension duration,blood glucose,cholesterol,carotid stenosis and total carotid plaque area(P<0.05),Excluding the course of hypertension,blood glucose level,cholesterol level and other factors,the degree of carotid stenosis and the total carotid plaque area were still risk factors for cerebral infarction(r=-0.369,r=-0.741,P<0.05).Conclusion:The degree of carotid artery stenosis and the total area of two carotid artery plaques are risk factors for patients with cerebral infarction.Clinicians should give corresponding preventive measures according to the carotid plaque size and shape in order to improve the quality of life after treatment.
