摘要
目的探讨经皮微创Henry入路掌侧锁定钢板内固定治疗桡骨远端骨折的临床疗效。方法回顾性分析2011年8月—2015年1月采用经皮微创Henry入路掌侧锁定钢板内固定治疗的40例桡骨远端骨折患者临床资料,男性25例,女性15例;年龄25~70岁,平均65.5岁。致伤原因:摔伤30例,道路交通伤10例。评估患者手术时间、术中出血量、骨折愈合时间、术后并发症、腕关节主动屈伸、旋转活动范围及上肢功能(DASH评分)等。结果术后随访12~18个月,平均16.5个月。平均手术时间(55.6±6.5)min,术中出血量(35.8±4.5)m L,骨折愈合时间(7.5±2.5)个月;腕关节掌曲(73.2±6.4)°,背伸(64.5±4.2)°,旋前(81.6±3.5)°,旋后(76.5±5.5)°;DASH评分(5.6±2.4)分。末次随访时,X线检查无内固定物松动、断裂等并发症发生。结论经皮微创Henry入路掌侧锁定钢板内固定治疗桡骨远端骨折,具有创伤小、出血少、手术时间短、恢复快等优点。
Objective To evaluate the efficacy of percutaneous minimal invasive Henry approach in treatment of distal radial fractures with palmar locking plate.Methods Between Aug.2011 and Jan.2015,40 cases of distal radial fractures who were treated by percutaneous minimal invasive Henry approach with palmar locking plate were included in this study.There were 25 males and 15 females,with an average age of 65.5 years(25-70 years).There were 30 cases of falling injury and 10 cases of traffic accident injury.The operation time,blood loss,fracture healing time,complications,active range of motion of wrist joint and DASH score were assessed.Results Patients were followed up for 12 to 18 months(mean,16.5 months).The average operation time was(55.6±6.5)min,the average intra-operative blood loss was(35.8±4.5)mL,and the average fracture healing time was(7.5±2.5)months.The average motion of wrist joint indicated flexion of(73.2±6.4)°,extension of(64.5±4.2)°,pronation of(81.6±3.5)°,and supination of(76.5±5.5)°.The average DASH score was(5.6±2.4).There were no complications such as fixation loosening or breakage.Conclusion Percutaneous minimal invasive Henry approach in treatment of distal radial fractures with palmar locking plate has advantages such as small trauma,little bleeding,short operation time,and fast recovery.
