摘要
目的通过测定胎龄31~42周新生儿的胎儿血红蛋白(HbF)水平,了解正常新生儿和窒息新生儿HbF水平随胎龄的变化情况,比较二者差异,初步提供不同胎龄新生儿HbF的参考数据,为临床判断高危新生儿提供可能依据。方法收集该科2016年1月至2017年3月住院的523例新生儿血样,标本均按照采血当天新生儿矫正胎龄进行分组,31周0 d到31周6 d标本纳入31周分组,以此类推。最终得到正常新生儿组标本378例,窒息新生儿组标本145例。由于41、42周窒息新生儿样本量不足,最终窒息新生儿组仅采集到31~40周标本。使用ABL90血气分析仪测定HbF百分比。结果正常新生儿随胎龄的增加,HbF百分比逐渐下降,而窒息新生儿下降不明显。34~40周窒息新生儿HbF水平明显高于同胎龄正常新生儿,差异有统计学意义(P<0.05)。结论窒息是导致HbF水平升高的高危因素,通过比对不同胎龄HbF参考数据可早期识别高危新生儿,具有一定的临床意义。
Objective By measuring the level of fetal hemoglobin(HbF)in neonates at31~42weeks gestational ages,it is aimed to know how HbF levels change at different gestational ages for normal neonates and asphyxia neonates.This preliminary study provides both reference data on HbF levels for neonates at different gestational ages and possible basis for clinical identification of high-risk neonates.Methods The blood samples from523neonates hospitalized in the department from January2016to March2017were collected.According to the age of neonatal orthodontic treatment on the day of blood sampling,all specimens were grouped,the specimens of31weeks0d to31weeks6d were included in31weeks group,and so on.The378blood samples in normal neonates group,the145blood samples in asphyxial neonates group.Due to the insufficient sample size of the asphyxial neonates at41,42weeks,the final asphyxial neonates group only collected samples from31to40weeks.Measuring HbF percentage by ABL90Radiometer Corperation.Results There was a gradual decrease in HbF levels for normal neonates with increasing gestational ages,while no obvious decrease for asphyxia neonates.The asphyxia neonates at34~40weeks gestational ages showed much higher HbF levels than that normal neonates of the same gestational age,the differences were statistically significant(P<0.05).Conclusion Asphyxia is a high?risk element causing elevated HbF levels.It is therefore of potential clinical significance by observing HbF levels at different gestational ages,which could possibly provide an early identification of high?risk neonates.
