摘要
目的评估基于阶段转变模型进行疼痛教育的干预效果,探讨社区慢性疼痛管理的方案,促进有效疼痛管理和应对策略的应用。方法对陕西省欠发达地区某市2个社区64例慢性疼痛患者进行问卷调查,对照组社区患者仅接受常规治疗,干预组社区患者在常规治疗基础上接受以阶段转变模型为指导的疼痛教育3个月,干预结束后,比较2组所处的疼痛自我管理阶段分布和疼痛应对策略应用情况。结果 64例患者干预前平均使用过(12.59±2.90)种疼痛应对策略,最多的使用了19种应对策略,最少使用了5种应对策略。经过3个月疼痛教育,干预组进入自我管理高级阶段(行动期和稳定期)的患者比例超过干预前及对照组,干预组患者较对照组患者使用了更多的疼痛应对策略,差异有统计学意义(P<0.05)。患者所处的疼痛自我管理阶段与一般资料中所患慢病数量存在相关性(P<0.05)。不同自我管理阶段的老年患者使用的疼痛应对策略比较,差异有统计学意义(P<0.05),所处的管理阶段越高,使用的应对策略数量越多。结论不同自我管理阶段的患者所使用的疼痛应对策略存在差异,在进行疼痛教育时,应根据患者所处的疼痛自我管理阶段进行个性化的疼痛教育。根据阶段转变模型进行疼痛自我管理教育可以促进患者进入更高的自我管理阶段,使用更多的疼痛应对策略。
Objective To evaluate the interventional effect of pain education based on the phase change model,to ex-plore the scheme of community chronic pain management,and to promote the use of effective pain management and coping strat-egies. Methods Sixty-four patients with chronic pain in 2 communities of Shaanxi Province underdeveloped area conductedthe questionnaire survey. The community patients in the control group received only routine therapy,while the community pa-tients in the intervention group received the 3-month pain education guided by the phase change model on the basis of conven-tional treatment. After intervention end,the pain self-management stage distribution and application situation of pain copingstrategies were compared between the two groups. Results Before the intervention,64 patients used the(12.59±2.90)kinds ofpain coping strategy,the maximal use number reached 19 coping strategies,and the least used the 5 kinds of coping strategy.After the 3-month pain education,the patients proportion of the intervention group entering the advanced stage of self manage-ment(action and stable period)exceeded that before the intervention and the control group,the patients in the interventiongroup used more coping strategies than the patients in the control group,and the difference was statistically significant(P〈0.05). The correlation existed in the pain self-management stage of the patients and the number of chronic diseases in generaldata(P〈0.05). The coping strategies used by elderly patients had difference among different self management stages(P〈0.05).The higher the management stage was,the more the coping strategies number. Conclusion The difference exists in pain cop-ing strategies used by the patients with different stages of self-management. In conducting the pain education,the individual-ized pain education should be carried out according to the patient′s pain self management stage.
