摘要
目的中药可以增加晚期胃癌化疗的疗效并减轻化疗的毒性，但具体作用机制尚不明确。本研究探讨健脾化毒方联合化疗对晚期胃癌患者组织基质金属蛋白酶9（matrix metalloproteinase, MMP-9）和血管内皮生长因子（ vascularendothelial growth factor, VEGF）表达的影响以及临床疗效。方法2015-01-05-2018-01-05济南市济钢医院肿瘤科治疗的晚期胃癌患者102例作为研究对象，采用替吉奥联合奥沙利铂化疗51例作为对照组（治疗期间2例死亡），每3周为1个周期，在对照组基础上加用健脾化毒方的51例患者作为观察组（治疗期间1例转外地），4个周期后评价疗效和不良反应，并观察治疗前后胃癌组织MMP-9和VEGF表达的变化。结果对照组有效率（RR）为40．8％、观察组为50．0%，两组差异无统计学意义，P〉0．05；对照组疾病控制率为（DCR）为57．1％、观察组为76．0％，两组差异有统计学意义，P〈0．05）；观察组与对照组治疗前胃癌组织MMP-9和VEGF表达差异无统计学意义，P〉0．05；治疗后两组胃癌组织MMP-9和VEGF表达均有明显降低，并且治疗后观察组胃癌组织MMP-9、VEGF积分降低更显著（P〈0．05），观察组白细胞减少、恶心、呕吐的发生率显著低于对照组。结论健脾化毒方联合化疗治疗晚期胃癌较单纯化疗可以显著降低胃癌组织MMP-9和VEGF表达，提高疾病控制率，并且具有增效减毒作用。
OBJECTIVE To investigate the effect of Jianpi Huadu Recipe on the expression of matrix metalloprotei- nase-9（MMP-9） and vascular endothelial growth factor（VEGF） in advanced gastric cancer. METHODS From January 5, 2015 to January 5,2018,102 patients with advanced gastric cancer who were treated in the Oncology Department of J inan Jigang Hospital were selected as the research objects. They were randomly divided into the control group and the observa- tion group according to the numerical table method. Both groups were given S-1 combined with oxaliplatin every three weeks. The observation group was given Jianpi Huadu recipe on this basis. After 12 weeks,the curative effect and adverse reactions were evaluated,and the changes of MMP-9 and VEGF expression in gastric cancer tissues before and after treat- ment were observed. RESULTS The effective rate（RR） of the control group and the observation group was 40. 80/00 and 50.0% respectively,and there was no significant statistical difference between the two groups （P〉0.05）. The disease control rate（DCR） of the control group and the observation group was 57. 1% and 76.0% respectively, and there were significant statistical differences between the two groups （P〈0.05）. There was no significant statistical difference be- tween the observation group and the control group in MMP-9 and VEGF expression before treatment （P〉0.05）. After treatment,the expression of MMP-9 and VEGF in gastric cancer tissues of both groups decreased significantly （P〈0. 05）. The MMP 9 and VEGF scores in the observation group were significantly lower than those in the control group （P〈0.01）. The leukopenia, nausea and vomiting in the observation group were significantly lower than those in the con- trol group. CONCLUSION Jianpi Huadu recipe combined with chemotherapy can significantly reduce the expression of MMP-9 and VEGF in gastric cancer tissues, improve the disease control rate, and have synergistic and attenuated effects compared with chemotherapy alon
