摘要
目的探讨奥曲肽联合凝血酶治疗肝硬化上消化道出血患者的有效性。方法选取肝硬化上消化道出血患者150例,随机分为奥曲肽组(采用奥曲肽治疗)、凝血酶组(采用凝血酶治疗)与联合组(采用奥曲肽、凝血酶联合治疗)各50例,对比3组患者的临床效果、止血时间、症状改善时间、住院时间与不良反应发生率。结果联合组患者治疗有效率显著高于奥曲肽组与凝血酶组(P <0. 05);联合组患者止血时间、症状改善时间、住院时间均显著优于奥曲肽组与凝血酶组(P <0. 05);联合组患者的不良反应发生率显著低于奥曲肽组与凝血酶组(P <0. 05),但奥曲肽组与凝血酶组的不良反应发生率差异无统计学意义(P> 0. 05)。结论采用奥曲肽联合凝血酶治疗肝硬化上消化道出血患者,治疗效果较单一用药显著提升,缩短了止血时间、症状改善时间与住院时间,还降低了不良反应发生率。
Objective To investigate the efficacy of octreotide combined with thrombin in the treatment of upper gastrointestinal bleeding in patients with liver cirrhosis. Methods A total of 150 patients with upper gastrointestinal bleeding in our hospital were randomly divided into octreotide group, thrombin group and combined group, with 50 cases in each group. The patients in the octreotide group were treated with octreotide, the thrombin group was treated with thrombin, and the combined group were treated with combination therapy. Clinical effect, hemostasis time, symptom improvement time, hospitalization time and adverse reaction rate were compared. Results Combined group had significantly higher efficacy than octreotide group and thrombin group（ P 〈0.05）. Combined group had better bleeding time, improvement of symptoms and hospitalization time than the octreotide group and thrombin group（ P 〈0.05）. The incidence of adverse reaction was slightly lower than that in the octreotide group and thrombin group（ P 〈0.05）, but no significant difference was observed in the octreotide group and thrombin group （ P 〉0.05）. Conclusion Octreotide combined with thrombin in treatment of liver cirrhosis with upper digestive has better treatment efficacy compared with single drug, reduce hemostasis time, symptoms improvement time and hospitalization time, and reduce occurrence of adverse reactions.
