摘要
目的了解丹东地区新发现艾滋病病毒Ⅰ型(HIV-1)阳性者的疾病进程,探讨不同疾病进程阶段蛋白印迹试验(WB)带型的特点。方法采用流式细胞仪检测CD4^+T淋巴细胞(简称CD4细胞)计数,根据CD4细胞计数结果进行临床分期,对不同疾病期感染者组的WB特异性条带检出率及带型进行统计分析。结果对2016—2017年新发现的175例HIV-1阳性者中,WB特异性条带gp160、gp120、gp41、p66、p31及p24检出率均为100%,p51为95.43%,p17为86.29%,p55为64.57%。p51检出率在3组间差异无统计学意义(P〉0.05),p17和p55在艾滋病(AIDS)期的检出率与原发感染组和感染中期组的差异均有统计学意义(P〈0.05)。WB带型共出现6种类型:(1)全带型(未缺失),(2)缺失p55,(3)缺失p55+p17,(4)缺失p55+p51,(5)缺失p17,(6)缺失p55+p51+p17,各类型所占比例分别为60.57%(106人)、22.29%(39人)、8.57%(15人)、3.43%(6人)、4.00%(7人)、1.14%(2人);(1)全带型出现率在AIDS期组、原发感染组和感染中期组的差异均有统计学意义(χ^2=10.817,P=0.001;χ^2=8.805,P=0.003),(2)次全带(缺失p55)出现率在3组间差异均无统计学意义(P〉0.05)。结论 env带型和pol带型的出现率不受疾病病程的影响;gag带型p55和p17的转阴可以作为疾病发展为艾滋病期的一个潜在判别依据。
Objective To understand the disease process of newly detected HIV-1 antibody positives and discuss the characteristics of Western Blot(WB)band patterns in different clinical stages.Methods We used flow cytometry to test CD4^+T lymphocytes counts(CD4 cells)of the 175 HIV positives detected between 2016-2017,and clinical stages were judged according to CD4 cells counts,and followed by statistical analysis of the positive rate and band patterns tested by WB in different clinical stages.Results Out of the 175 cases of newly diagnosed HIV positives,the detection rates of WB-specific bands including gp160,gp120,gp41,p66,p31 and p24,were all 100%.The positive rate of bands p51,p17 and p55 were lower,accounting for 95.43%,86.29%and 64.57%.The positive rate of p51 had no statistical difference among the three stages(P〉0.05).The positive rates of p17 and p55 in the AIDS period had statistical difference among the primary infection stage and the middle infection stage(P〈0.05).There were six types of WB bands,i.e.total bands(not missing),missing p55 band,missing p55 + p17 bands,missing p55+p51 bands,missing p17 band,and missing p55+p51+ p17 bands,accounting for 60.57%(106/175),22.29%(39/175),8.57%(15/175),3.43%(6/175),4.00%(7/175),and 1.14%(2/175).There were statistically significant differences in the prevalence of full-band AIDS between the primary infection group and the middle infection group(χ^2=10.817,P=0.001,χ^2=8.805,P=0.003),and there were no statistical differences of subtotal bands among the three stages(P〉0.05).Conclusion The positive rate of env band and pol band are not affected by the stages of the disease.If gag band p55 and p17 turn to be negative,we can use it as a potential basis to diagnose the development of AIDS.
