摘要
目的探析获得性免疫缺陷综合征(AIDS)患者肺部感染的影像学诊断价值。方法选取2012年1月-2016年1月医院收治的HIV合并肺部感染患者389例,对其进行实验室检查及CT检查,分析HIV患者肺部感染情况及影像学表现。结果 389例患者中,以年龄18~29岁176例为主,占45.2%;肺部感染类型以肺结核199例为主,占51.2%;199例肺结核患者中,168例患者AIDS期具有多种多样的肺结核病变形态,其余31例具有较轻的X线征象,5例患者为中等量胸腔积液;117例肺孢子菌患者中,典型X线表现为毛磨玻璃样浸润及对称性网状或网织结节状影弥散于两侧肺门周围,全肺会在几天内受到累及,促进弥漫性实变的发生,但是仍然有网状影出现在肺野外围。结论 HIV合并肺部感染的影像学诊断价值高,CT检查特别是高分辨力CT能够有效表现病灶特征,为临床诊断和治疗工作提供依据,从而改善患者预后,值得临床推广使用。
OBJECTIVE To explore the value of imaging diagnosis of pulmonary infections in patients with acquired immunodeficiency syndrome（AIDS）.METHODS A total of 389 HIV patients complicated with pulmonary infec-tions who were treated from Jan 2012 to Jan 2016 were enrolled in the study.The laboratory examination and CT examination were carried out,and the incidence of pulmonary infections and imaging manifestations of the HIV pa-tients were analyzed.RESULTS Of the 389 patients,176 were aged between 18 and 29 years old,accounting for 45.2%;among the patients with pulmonary infections,199 had pulmonary tuberculosis,accounting for 51.2%. Of the 199 patients with pulmonary tuberculosis,168 showed diversified tuberculosis lesions forms during the AIDS stage,the rest of 31 patients showed mild X-ray signs,and 5 patients had moderate pleural effusion.Among the 117 patients with sporotrichosis,the hairy groundglass infiltration and the symmetry reticular or nodular shad-ow dispersed around the bilateral hilus pulmonis were the typical X-ray manifestations,the whole lung was in-volved within a few days,which promoted the occurrence of diffuse consolidation,however,the reticular opacities still appeared in the lung periphery.CONCLUSION The value of the imaging diagnosis is high for the HIV patients complicated with pulmonary infections;CT,especially high resolution CT,may effectively show the characteris-tics of focus and provide guidance for clinical diagnosis and treatment so as to improve the prognosis of the pa-tients,and it is worthy to be promoted in the hospital.
