摘要
目的：探讨文拉法辛缓释片联合丁螺环酮治疗伴焦虑症状抑郁症的效果。方法：随机数表法将84例伴焦虑症状的抑郁症患者分为联合组与对照组各42例,分别予以单用文拉法辛缓释片或联合丁螺环酮治疗。比较治疗前及治疗6周后,两组患者绝望程度[Beck绝望量表（BHS）]、自杀倾向[自杀意念评定量表（SIOSS）]、神经营养状态[脑源性神经营养因子（BDNF）、神经生长因子（NGF）]及炎性因子水平[干扰素-γ（INF-γ）、白细胞介素-9（IL-9）、白细胞介素-18（IL-18）]变化,分析治疗6周内两组患者药物不良反应[治疗副反应量表（TESS）]评分差异。结果：治疗6周后,两组患者BHS、SIOSS评分及NGF、INF-γ、IL-9、IL-18水平均较治疗前有显著下降（P〈0.05）,且联合组明显低于同期对照组（t=-6.609,-5.703,-8.097,-3.159,-2.973,-2.541;P〈0.05）;两组患者BDNF水平均较治疗前有显著提升（P〈0.05）,且联合组明显高于同期对照组（t=5.191;P〈0.05）。治疗6周内,两组患者TESS总分比较无统计学意义。结论：文拉法辛缓释片联合丁螺环酮治疗伴焦虑症状抑郁症效果确切,对患者预后康复有利。
Objective：To explore the effects of venlafaxine sustained-release tablets and buspirone for depression with anxiety symptoms.Methods：A total of 84 depression patients with anxiety symptoms were randomly divided into the combined group and the control group（ each n = 42）,and were given single venlafaxine sustained-release tablets,or combined with buspirone respectively. The despair degree[Beck desperation scale（ BHS）],the suicidal tendencies[suicide ideation rating scale（ SIOSS）],the neurotrophic state[brain-derived neurotrophic factor（ BDNF）,nerve growth factor（ NGF）]and inflammatory factors levels[interferon-γ（ INF-γ）,interleukin-9（ IL-9）,interleukin-18（ IL-18）]were compared before treatment and after 6 weeks of treatment,and the occurrence of adverse drug reactions[treatment emergent symptom scale（TESS）]was analyzed within 6 weeks of treatment.Results：After 6 weeks of treatment,the scores of BHS,SIOSS,and the levels of NGF,INF-γ,IL-9 and IL-18 in the two groups were significantly lower than those before treatment（ all P〈0.05）,and the indexes levels in the combined group were lower than those in the control group at same period（ t =-6.609,-5.703,-8.097,-3.159,-2.973,-2.541;P〈0.05）.The BDNF level was significantly increased compared with that before treatment（ P〈0.05）,and the level in the combined group was significantly higher than that in the control group at the same period（ t = 5.191,P〈0.05）.Within 6 weeks of treatment,there was no difference in the total TESS score in the two groups. Conclusion：Venlafaxine sustained-release tablets combined with buspirone can have an exact effect on depression with anxiety symptoms,and it is favorable for the prognosis and rehabilitation.
